摘  要:目的:观察Affinity系列和Kappa 700系列起搏器自动夺获(AC)功能的不同特点。方法:对置入Kappa700系列43例和Affinity系列30例起搏器进行术中测试和术后6个月的随访,程控监测其AC功能的工作状况。结果:起搏阈值:在术中,术后1个月、3个月和6个月时,Kappa700系列分别为(0.37士0.02)、(0.87士0.02)、(0.63士0.02)和(0.53士0.04)V,Affinity系列分别为(0.36±0.08)、(0.69±0.17)、(0.81±0.14)和(0.68±0.11)V。心室起搏输出:在术后1个月、3个月和6个月时,Kappa 700系列分别为(2.25士0.72)、(1.86土0.41)和(1.55士0.22)V,Affinity系列分别为(1.17±0.16)、(1.05±0.15)和(0.91±0.13)V。AC功能:在Kappa 700系列,只需将起搏器的心室夺获管理设定为"Adaptive"即可开启,再设定阈值的安全范围和最小适应性电压后,即能正常工作;在Affinity系列,则需另行测试ER信号、极化电位和ER感知灵敏度等,并满足相关要求,方能开启AC功能并使其正常工作。结论:Affinity系列起搏器节能精确,但程控操作复杂;Kappa 700系列起搏器使用简便,更适合于不便紧密随访的患者。Objective:To study the different features of pacemakers with auto-capture (AC) function in Kappa 700 series and Affinity series. Method.. The relevant parameters were tested in the procdeure and six-month followup in patients implanted with Kappa 700 series pacemakers (n= 43) and patients with Affinity series (n= 30). The status of AC function were measures. Result:Pacing threshold in procedure 1 m,3 m and 6 m after procedure in Kappa700 series were (0.37 ± 0.02)V, (0.87 ± 0. 02)V,(0. 63 ± 0.02)V and (0.53 ± 0.04) V respectively, and in Affinity series, were (0.36±0. 08) V, (0.69±0.17)V, (0.81±0.14)V and (0.68±0. 11)V respectively. Pacing output in Kappa 700 series, 1 m,3 m and 6 m after operation were (2.25 ± 0.72) V, (1.86 ± 0.41) Vand (1.55±0.22)V respectively, and in Affinity series, were (1.17±0.16)V, (1.05±0.15)V and (0.91±0.13)V respectively. AC function in Kappa 700 series began when "Adaptive" in VCM was selected and worked normally after threshold safety margin and minimum adaptive voltage were selected. In Affinity series, before AC function normally worked, ER, polarized potentials and ER sensitivities should be tested in addition and their requirements should be met. Conclusion: Affinity series pacemakers has exact reduction in energy output and more complex programmable procedures. Kappa 700 series pacemakers are easier to handle and more suitable for the patients who are not within easy reach of close follow-up.
