摘 要:目的探讨细胞间黏附分子-1(ICAM-1)及其配体淋巴细胞功能相关抗原-1(LFA-1)对热性惊厥(FS)患儿的神经免疫调节作用。方法40例FS患几分为单纯性FS(SFS)组20例和复杂性FS(CFS)组20例;对照组19例。分别采用实时荧光定量PCR、RT-PCR方法检测外周血单个核细胞(PBMC)ICAM-1/LFA-1 mRNA的表达水平。结果SFS组患儿ICAM—1 mRNA水平为(0.63±0.39)基因copy数,明显高于对照组(0.46±0.20)和CFS组患儿(0.41±0.20)基因copy数(P〈0.05);CFS组ICAM—1 mRNA水平与对照组儿童比较有下调趋势,但差异无统计学意义(P〉0.05)。SFS组LFA—1 mRNA表达水平为(0.73±0.11)灰度值比。明显高于对照组(0.60±0.15)和CFS组(0.54±0.17)灰度值比(P〈0.05);CFS组患儿LFA-1 mRNA水平与对照组儿童比较亦有下调趋势,但差异无统计学意义(P〉0.05)。结论PBMC ICAM-1/LFA-1基因表达水平在SFS和CFS是不同的,ICAM-1/IFA-1 mRNA表达增高所介导的炎性免疫病理损害在SFS患儿发病机制中有一定的作用,而CFS患儿ICAM-1/LFA-1 mRNA表达反而下调可能有一定的神经保护作用。Objective To explore the neuroimmunomodulation mechanism of ICAM-1 and LFA-1 in children with febrile seizures (FS). Methods 40 children with FS were divided into simple FS (SFS) group in 20 cases and complex FS (CFS) group in 20 eases, and 30 health children matched with regard to age and sex were enrolled into control group. The real-time fluorescence quantitative PCR was used to detect the expression of PBMC ICAM-1 mRNA. At the same time, the PBMC LFA-1 mRNA expression was studied with Semi-Quantitative RT-PCR analysis. Results The levels of PBMC ICAM-1 mRNA in SFS group were significantly higher than those in control group and CFS group ( P 〈0. 05 ). The levels of PBMC ICAM-1 mRNA showed downtrend between CFS group and control group, but there was nostatisti- cal difference between the two groups ( P 〉 0.05 ). The levels of PBMC LFA-1 mRNA grey-scales in SFS group were significantly higher than those in control group and CFS group ( P 〈 0. 05 ). In addition, the levels of PBMC LFA-1 mRNA in CFS group showed downtrend than those in control group, but there was no statistical difference between the two groups ( P 〉 0. 05 ). Conclusions The gene expression levels of PBMC ICAM-1/LFA-1 in SFS group were different from those in CFS group. Inflammable immunopathology damage induced by ICAM-1/LFA-1 may play an important role in the pathogenesis of SFS. On the contrary, ICAM-1/LFA-1 may have some neuroprotective effects on the pathogenesis of CFS.
