摘  要:目的探讨先天性心脏病(简称先心病)患儿血清可溶性细胞黏附分子-1(sICAM—1)、可溶性P选择素(SPS)、肌钙蛋白Ⅰ(cTnI)在介入治疗前后的变化及其临床意义。方法2005年7月至2008年2月山东大学齐鲁医院和济南市中心医院施行先心病室间隔缺损(VSD)介入封堵治疗患儿54例,房间隔缺损(ASD)介入封堵治疗患儿60例,用酶联免疫吸附法(ELISA)检测患儿血清中sICAM—1、SRS、cTnI在介入封堵治疗术前后的浓度变化。结果与术前相比,术后即刻的siCAM-1、SPS均显著升高(P〈0.05);术后24h、48h的sICAM—1、SPS下降,其中48h达最低水平(P〈0.05),术后72h二者又显著升高(P〈0.05);所有cTnI值均在正常范围。结论介入治疗VSD、ASD可以激活siCAM-1、SPS的表达。Objective To study the changes and significance of serum soluble intercellular adhesion molecule-1 (sICAM-1), soluble P-selectin (sPs) and troponin I (cTnI) in patients with ventricular septal defect(VSD) and atrial septal defect(ASD) after transcatheter closure .Methods Using enzyme-linked immunosorbent assays ( ELISA ), serum concentrations of soluble ICAM-1, soluble P-selectin and troponin I were measured before and after transc.atheier closure. Results The serum sICAM- 1, sPs rose significantly immediately at the end of closing procedure ( P 〈 0.05 ) . The serum sICAM-1, sPs, cTnI decreased 24 h and 48 h after procedure, decreased to the baseline 48 h after procedure (P 〈 0.05, compared with pre-procedure), increased significantly 72 h after procedure (P 〈 0.05, compared with pre-procedure ). But all the serum levels of cTnl were in normal range. Conclusion Transcatheter closure of ventricular septal defect activate the expression of sICAM-I and SPS, but do not result in visible myocardial damage.
