原位杂交技术检测与中药干预治疗宫颈高危型HPV感染的对比研究

摘　　要：目的:探讨原位杂交技术(ISH)检测宫颈高危型HPV(16/18型)感染与宫颈癌及癌前病变发生的相关性,分析中药干预和治疗HPV16/18型感染的临床价值。方法:运用原位杂交技术检测218例宫颈组织HPV16/18型的表达水平,判定HPV16/18型感染对宫颈癌及癌前病变的诊断价值;应用中药清热解毒扶正方干预和治疗HPV感染患者,并观察用药前后变化。结果:218例患者HPV16/18检出率分别为:CINⅠ16.67%(15/90例)、CINⅡ48.43%(31/64例)、CINⅢ91.11%、(41/45例)、SCC84.21%(16/19例);实验组52例,对HPV感染者用清热解毒、活血化瘀方剂局部熏蒸外洗和益气温阳、清热扶正方剂水煎口服,显效率34.62%(18/52例);对照组51例,显效率11.54%(6/51例),两组间比较有显著性差异(P<0.01)。结论:原位杂交技术检测HPV16/18型对诊断宫颈癌及癌前病变具有早期、敏感、特异的特点;中药清热解毒扶正方治疗HPV感染者疗效显著,若二者协同使用对宫颈癌的早期筛查和干预治疗,具有较高的临床使用价值和推广前景。Objective：To explore the relativity of the in situ hybridization detection for cervical high-risk HPV （16/18 type）and cervical cancer and precancerous lesions. And to analyse the effect of intervention by traditional Chinese medicine. Methods ： Giving the high-risk HPV-DNA detection to 218 female eases. The patients infected by HPV（16/18） were treated by Clearing away Heat and Reinforcing Body Resistance Prescription. Results：The positive rate of HPV-DNA （ 16/18 ） was as follows： 16. 67% in CINI（15/90） ,48.43% in CINII （ 31/64 ） ,91.11% in CINIII （41/45）, 84. 21% in SCC （ 16/19 ）. 52 cases in trail group received local fumigation to clear away heat and enrich qi and blood, together with taking Qi-invigorating and Reinforcing Body Resistance Prescription. The significant effeiency was 34. 64% （18/52）. 51 cases was in control group,and the significant effeiency was 1 1.54% （6/51）. There was a significant difference between thetwo groups （P 〈 0. 01 ）. Conclusion：In situ hybridization detection of HPV （ 16/18 type）was early, accurate, specific to detect cervical cancer and precancerous lesions. The effect of Clearing away Heat and Reinforcing Body Resistance Prescription was distingwished. It showed a high clinical value in improving cervical cancer early screening by in situ hybridization and intervention by traditional Chinese medicine to use them synergistically.
