β-阻滞剂在慢性心力衰竭患者中的应用

摘　　要：目的了解慢性心力衰竭病例β-受体阻滞剂的应用情况。方法对慢性心衰患者应用β-受体阻滞剂的情况进行问卷调查。结果入选1 238例,门诊病例427例,住院病历811例。临床诊断依次是冠心病（43.8%）、高血压性心脏病（16.7%）、风湿性心脏病（14.7%）、肺心病（8.8%）、扩张型心肌病（6.8%）等。入选病例心功能以纽约心功能分级（NYHA）Ⅱ~Ⅳ级。治疗药物在应用硝酸酯类、利尿剂、洋地黄制剂的基础上应用β-受体阻滞剂,β-受体阻滞剂的应用无性别和年龄差异。结论慢性心衰住院病例的治疗药物传统以利尿剂、硝酸酯制剂和洋地黄制剂等为主,随着临床研究的发展,β-受体阻滞剂的应用逐年增加,但仍未达到＂指南＂的要求,须不断更新治疗观念,使更多的心衰患者受益。Objective To investigate the application of β - receptor blocker in chronic heart failure patients. Methods the application of β- blockersm patients with chronic heart failure was conducted the survey. Results Total 1238 cases selected, 427 cases of out - patient, 811 cases of in - patient medical records, clinical diagnosis of coronary heart disease （43.8%）, high blood pressure heart disease （ 16.7% ） , rheumatic heart disease （ 14.7% ） , pulmonary heart disease （ 8.8% ）, dilated cardiomyopathy （6. 8% ）. Selected cases cardiac function were graded in heart function classification of New York （NYHA） Ⅱ -Ⅳ grade level, on the basis of the application of nitrates, diuretics, digitalis preparations applly β- receptor blocker, without gender and age differences. Conclusion The patient with chronic heart failure mainly treated with conventional drugs diuretics, and digitalis preparations nitrate - based agents, and so on, with the development of clinical research, β - receptor blocker applications has increased year by year, but have not yet reached the ＂Guide＂ standard, The concept of treatment has to he updated for more benefit in patients with heart failure.
