摘　　要：目的应用超声多普勒Tei指数评价Ⅱ型糖尿病患者左室功能;探讨高血压对糖尿病患者心脏功能的影响。方法对Ⅱ型糖尿病患者45例(DM组),糖尿病合并高血压患者35例(DM+HP组),正常对照组40例(NC组)常规行超声心动图检查并测量左室Tei指数、左室等容舒张时间(IRT)、左室等容收缩时间(ICT)、IRT/ET、ICT/ET及左室射血分数(EF),分别比较各组参数。结果DM组与NC组、DM+HP组与NC组,DM+HP组与DM组比较,左室Tei指数均升高(P<0.01);而DM组与NC组、DM+HP组与NC组比较,还有IRT、IRT/ET明显升高,E/A值明显降低(P<0.01);DM+HP组与DM组比较,ICT、ICT/ET升高,EF值明显降低(P<0.01);左心Tei指数与ICT、IRT、ICT/ET及IRT/ET呈正相关,与EF值呈负相关,与E/A值无相关(P>0.05)。结论Tei指数能够简便、有效、综合地评价糖尿病患者心脏收缩与舒张功能,高血压可加重糖尿病患者心功能的损害。objective To evaluate the left ventricular function in patient with type 2 diabetes by Tei-index and investigate the effectiveness of hypertension on left ventricular function in patients with type 2 diabetes.Methods Eighty patients with type 2diabetes and forty cases of healthy control were divided into three groups：DM, DM＋HP and NC.Echocardiography was used to evaluate the left ventricular structure and function.Left ventricular Tei-index,LV-IRT, LV-ICT, ICT/ET, IRT/ET, LVEF and E/A ratio were measured and compared among healthy group,DM group and DM＋HP group.Results Tei-index weresignift cantly higher, IRT, IRT/ET were prolonged and E/A were reduced in group DM and group DM＋HP than those in group NC（P〈0.01）. Tei-index were significantly higher, ICT, ICT/ET, EF were reduced in group DM＋HP than those in group DM （P〈0.01）.Left ventricular Tei-index was positively correlated with IRT, ICT, ICT/ET, IRT/ET and was negatively correlated with Left ventricular EF and was non-correlated with E/A ratio.Conclusion Tei-index can easily effectively, synthically assess the left ventricular systolic and diastolic function in patients with type 2 diabetes and Hypertension aggravates the function of left ventricular in patients with type 2 diatetes.
