摘 要:目的对吉林大学第一临床医院2007年全年临床标本分离出来的革兰染色阴性病原菌构成及对抗生素的耐药率做一次回顾性分析,为临床合理使用抗生素提供科学依据。方法对临床分离出来的849株革兰染色阴性菌株用Kirby-Bauer法进行药敏试验。结果849株革兰阴性细菌中,肠杆菌科细菌占58.8%,非发酵革兰阴性杆菌占33.2%。大肠埃希菌和肺炎克雷伯杆菌中ESBLs的检出率分别为60.4%和64.2%。除碳青霉烯类,ESBLs对哌拉西林/他唑巴坦,阿米卡星,头孢西丁,环丙沙星和左氧氟沙星的耐药率较低。非发酵菌对亚胺培南,美洛培南,左氧氟沙星,哌拉西林/他唑巴坦的耐药率较低。结论加强细菌耐药性监测,对合理使用抗菌药物,减少耐药菌株的产生和流行有重要临床指导价值。Objective To investigate the situation of the isolated gram negative organisms from the clinical department in the first hospital of JiLin University in 2007 and to provide suggestions empirical treatment against the bacteria. Methods Anfimicrobial susceptibility test were conducted by Kirby-Bauer method.Results. Of the 849 gram negative organisms, enterobaeteriaceae aeceuntod for 58.8% ,non-fermenters 33.2%. Alxmt 60.4% of E.eoli isolates and 64.2% of Klebsiella isolates produced extended-spectrumβ- tamases (ESBLs). Except for earbopenems, ESBL( s ) was sensitive to piperacillin-tazobaetam., amikaein, cefoxitin, ciprofloxaein and Levofloxaein. The resistance rate of non-fermenters was lower to imipenem, meropenem, levofloxaein, piperaeillin-tazobaetam. Conclusion Surveillance of bacterial resistance is of great importance for rational use of antibiotics and reducing the emergence of resistance.
