生脉注射液联合50%葡萄糖治疗老年透析相关性低血压

摘　　要：目的:观察生脉注射液联合50%葡萄糖注射液治疗老年血液透析相关性低血压的效果。方法:选择反复发生低血压的维持性血液透析的老年慢性肾衰竭35例,将35例随机分为两组,治疗组18例,对照组17例。两组均在常规血液透析基础上加用50%葡萄糖100 mL持续静脉滴注,治疗组在此基础上联合应用生脉注射液50 mL持续静脉滴注。观察两组血压、心率的变化情况及治疗效果。结果:治疗总有效率:治疗组为88.9%,对照组为64.7%,两组比较差异有统计学意义(P<0.05);透析2小时治疗组收缩压、舒张压、平均动脉压均高于对照组,两组比较差异有统计学意义(P<0.01);治疗组透析后平均动脉压较对照组高,两组比较差异有统计学意义(P<0.05)。两组透析前后心率均无明显变化。结论:对透析过程中反复发生透析相关性低血压的老年患者,透析开始后即予生脉注射液联合50%葡萄糖注射液维持静脉滴注,可明显降低低血压的发生率,提高患者对透析的耐受性,且无不良反应发生,避免了因发生低血压提前结束透析的情况。Objective：To observe the clinical curative effect of the Shengmai injection liquid and Hypertonic sugar liquid used to treat dialysis hypotension in elder patients. Methods：35 elder patients with dialysis hypotension were randomly divided into two groups;17 in the control group,and 18 in the treatment group. We treated each patient in the control group with normal dialysis and 50% of Hypertonic sugar injection liquid （100 mL） and with the Shengmai injection liquid （50 mL） in the treatment group iv gtt during dialysis. The clinical curative effect,blood pressure and heart rate were recorded. Results：The curative rate in the treatment group was 88.9% ,and 64. 7% in the control group with statistical significance （P 〈 0. 05）. The systolic blood pressure, diastolic blood pressure and the mean arterial pressure in the treatment group were all higher than those in the control group two hours after dialysis with significant difference （P 〈 0. 01 ）. The variation of the mean arterial pressure after dialysis was higher in the treatment group （P 〈 0.05）. There was no significant difference of the heart rate before and after the dialysis between the two groups. Conclusion：The combined therapy can reduce the occurrence rates of hypotension and improve resistance to dialysis. Therefore termination ahead of time of dialysis because of hypotension can be avoided.
