不同丙泊酚复合用药用于无痛人工流产麻醉效果比较

摘　　要：目的比较丙泊酚联合氯胺酮、芬太尼和曲马多的麻醉效果,探讨无痛人工流产麻醉药物的最佳配伍。方法自愿行人工流产者150例,随机分成3组,每组50例。A组:丙泊酚200mg+氯胺酮50mg的混合液;B组:丙泊酚200mg+芬太尼0.05mg的混合液;C组:丙泊酚200mg+曲马多100mg;静脉给药。观察三组病人术前、术后的血压(MAP)、呼吸(RR)、心率(HR),氧饱和度(SpO2)及所用丙泊酚的量和清醒时间。结果A组定向力恢复时间长于B、C组(P>0.01),B组循环呼吸抑制较氧胺酮、曲马多组明显(P>0.05)。结论丙泊酚联合氯胺酮、芬太尼和曲马多的麻醉效果各有特点,根据具体情况来选择合适的麻醉方法。Objective To compare the effect of ketamine, fentanyl and tramadol combined with propofol for painless artificial abortion in order to study the best compatibility of anesthetics. Methods A total of 150 patients were allocated into 3 groups, patients in Group A （ n = 50） were applied with mixture of propofol 200 mg＋ketamine 50mg, patients in Group B（ n = 50） were applied with mixture of propofol 200mg ＋ fentanyl 0.05 mg and patients in Group C （ n = 50 ） were applied with mixture of propofol 200mg ＋ tramadol 100mg by intravenous administration. MAP, RR, HR and SPO2, amount of prepofol and recovery time before and after operation were observed in patients of these 3 groups. Results The recovery time of patients in group A was longer than those of other groups, and the inhibition of respiration and circulation was obviously lower than ketamine and fentanyl in patients of group B （ P 〉 0.05 ）. Conclusion Ketamine, fentanyl and tramadol combined with propofol have different characteristics, and you may choose the best combination according to concrete condition.
