丹参酮对女性迟发性痤疮患者血清性激素水平的影响及疗效观察

摘　　要：目的探讨丹参酮对女性迟发性痤疮患者血清性激素水平的影响及临床疗效。方法选择门诊女性迟发性痤疮患者115例,随机分为:治疗组(A组)60例,口服丹参酮胶囊4粒,4次/d;对照组(B组)55例,口服多西环素片0.1 g,2次/d,4周后减为0.1 g,1次/d。8周为一疗程,疗程结束后进行疗效评价。并对治疗组(A组)患者,于治疗前后分别采用电化学发光免疫分析法检测其卵泡期血清六项性激素水平,另选40例相应年龄段的健康女性作健康对照组(C组)。结果A组痊愈率为36.7%,有效率为78.4%;B组痊愈率为14.5%,有效率为45.4%,A组痊愈率及有效率均高于B组,差异有统计学意义(P<0.05或<0.01)。A组女性迟发性痤疮患者治疗前血清睾酮水平较C组明显升高(P<0.05),而治疗后血清睾酮水平与C组相比差异无统计学意义(P>0.05)。结论女性迟发性痤疮发病的主要原因与雄激素分泌增多有关。丹参酮具有抗雄激素作用,对女性迟发性痤疮的疗效确切,且无耐药性及明显不良反应。Objective To investigate the levels of serum sex hormone changes and the therapeutic effect of tanshinone on treating late-onset female patients with acne vulgaris.Methods 115 late-onset female patients with acne vulgaris were randomly divided into two groups, group A： 60 patients were given 4 grains tanshinone capsule, 4 times a day, PO; group B： 55 patients were given doxycycline 0.1 g, 2 times a day, then 0. 1 g, 1 times a day, PO, after 4 weeks, 8 weeks as a course, and the therapeutic effect analysis was taken after the therapy. For group A, we also detected the levels of serum sex hormone changes before and after the treatment and compared with that of the normal controls （group C） respectively. Results In group A, the cure rate was 36.7 %, and total effective rate was 78.3 % ; in group B, the cure rate was 14.6%, and total effective rate was 45.4%, There were significant differences between the two groups both the cure rate （ P 〈 0.05） and total effective rate （ P 〈 0.01 ）. The serum level of testosterone before the treatment was significantly higher than that of the normal controls （ P 〈 0.05）. No significant difference was found in the serum levels of testosterone between the female patients with acne after the treatment and that of the normal controls （ P 〉 0.05）. Conclusion The level of serum testosterone may be increased in female patients with late-onset acne vulgaris. The tanshinone acts antagonistically to testosterone and it is effective and safe in treatment of late-onset female patients with acne vulgaris, no resistance and obvious side effects were observed.
