中医药治疗肝炎后早期肝硬化疗效的系统评价

摘　　要：目的:运用循证医学的方法对中医药治疗肝炎后早期肝硬化的疗效进行系统评价。方法:检索并查阅2000年1月至2008年1月期间以肝炎后早期肝硬化患者为研究对象,设有中医或有中西医结合治疗组的临床研究文献,采用Cochrane系统评价方法,选取符合标准的临床研究文献资料,进行Meta统计分析。结果:入选文献共6篇,总病例数432例。Meta分析显示,中医或中西医结合治疗疗效在肝炎后早期肝硬化的总有效率、肝纤维化、肝功能等指标方面明显优于单纯常规西医药对照组(P<0.01)。结论:文献分析显示中医或中西医结合治疗方案在治疗肝炎后早期肝硬化方面具有一定的优势,但高质量的中医研究文献较少,提示今后在研究的方法学上有待进一步完善。Objective ： To systematically review the therapeutic effects of traditional Chinese medicine （TCM） on posthepatitic cirrhosis at early stage by the way of evidence-based medicine（EBM）. Methods： Retrieving and reading the clincial research literature regarding patients of posthepatitic cirrhosis at early stage and involving the group of TCM treatment or group of integrated treatment which were issued between January 2000 and January 2008. The literature was evaluated with Cochrane system , and the one met the requirement underwent recta-analysis. Results ： A total of 432 cases of posthepatitie cirrhosis were involved in the 6 articles. The recta-analysis showed that the TCM treatment or integrated treatment was superior to the pure western medicine treatment in total response rate, liver fibrosis indexes and liver functions indexes（P 〈 0.01 ）. Conclusion： The analysis of literautre shows that the TCM treatment or integrated treatment have advantages over pure western medicine treatment in treating posthepatitic cirrhosis at early stage. The fact that there is less clincial research literature of high quality indicates that the research methodology should be improved further.
