摘 要:目的:研究医院重症监护病房(ICU)、烧伤病房环境中耐甲氧西林金黄色葡萄球菌(MRSA)的分子流行病学特征。方法:采用脉冲场凝胶电泳(PFGE)分型技术对MRSA进行同源性分析,K-B纸片扩散法进行药敏试验。结果:28株MRSA的PFGE指纹图谱显示7个基因型(A-G),以A型(8株)和E型(6株)为主,分别来自烧伤病房和ICU。其对青霉素、氨苄青霉素、阿莫西林、苯唑西林、红霉素、环丙沙星、诺氟沙星、氧氟沙星等8种抗生素耐药率均为100%,对头孢西丁、庆大霉素、四环素、氯霉素、复方新诺明耐药率分别为96.4%、67.9%、64.3%、42.9%、14.3%,仅对万古霉素100%敏感。结论:MRSA耐药性较为严重,其PFGE基因分型阐述了烧伤病房和ICU分别存在MRSA单克隆流行趋势。Objective:To investigate the characteristic of the molecular epidemiology and antibiotic resistance of Methicillin-resistant Staphylococcus aureus(MRSA) isolated from intense care unit(ICU) and burns wards.Methods:Pulsed-field gel electrophoresis(PFGE) and Kirby-Bauey method were used to analyze the homology and drug-resistance of MRSA strains,respectively.Results:There were seven genotypes(A-G) in 28 strains of MRSA,and genotype A(eight strains) and E(six strains) were isolated from burnt wards and ICU.To penicillin,ampicillin,amoxicillin,oxacillin,illotycin,ciprofloxacin,norfloxacin,ofloxacin,the resistance rates of MRSA were all 100%.And to cefoxitin,cidomycin,ambramycin,amphemycin,trimethoprim-sulfamethoxazole,the resistance rates of MRSA were 96.4%,67.9%,64.3%,42.9%,14.3%,respectively.But the sensitivity of MRSA to vancomycin was 100%.Conclusion:The resistance of antibiotic is a serious problem in MRSA,and result of PFGE elucidates that monoclone prevalent trend may exist in ICU and burns wards.
