摘 要:[目的]探讨异氟醚低流量循环紧闭麻醉后达到最佳麻醉效果的方法.[方法]选择40例无心肺疾病,ASA分级Ⅰ,Ⅱ级的择期手术病人,随机分为A,B两组,均实施快速诱导气管内插管静脉吸入复合全身麻醉.A,B两组氧流量分别为0.8,2.0L/min,异氟醚吸入浓度为40mL/L,连续监测异氟醚吸入浓度(Ci)和呼出浓度(Ce),并观察异氟醚肺泡浓度升高速率、差别、达平衡所需时间(Te)及平衡时浓度(Cmax),用直线回归法求出Ce与蒸发器开启浓度(Cn)间的相关系数R及回归方程.[结果]A,B两组Ce/Ci值均随时间延长而增高,且Ci,Ce与时间在一定范围内呈线性相关.A,B组回归方程分别为Y=0.443+0.015 77X及Y=1.043+0.020 67 X.从其中求出A,B两组达到平衡所需时间分别为(7.2±0.3)min,(4.9±0.2)min.[结论]即使低氧流量异氟醚吸入麻醉,亦可达到理想的麻醉效果.OBJECTIVE To investigate low-flow closed circuit anesthesia (LFCCA) with isoflurane to find an ideal technique on low-flow inhalator anesthesia. METHODS Forty patients (ASAⅠ ,Ⅱ ) were randomly divided into groups A and B. Anesthesia were performed with rapid induction intubation and maintained with 40 mL/L isoflurane at oxygen flow rate of 0. 8 L/min and 2.0 L/min, respectively. The Ci and Ce were monitored continuously, the rising speed, the difference of alveolar concentration and the time (Te) and concentration (Cmax) were recorded as soon as Ci and Ce got equilibrium. The relative coefficient and regression equation between Cmax and Cn were work out by linear logical regression. RESULTS The ratio of Ce/Ci both were increased in company with Te in groups A and B. There was a linear correlation between Ci and Ce . The lineal regression equation in group A and B were Y= 0. 443+ 0. 015 77X and Y=1. 043+0. 020 67 X, respectively. The calculated Te in group A and 13 were (7.2±0.3) rain and (4.9 ± 0.2 )min, respectively. CONCLUSION During low-flow isoflurane anesthesia, the concentration of the alveoli and the Ci can acquire balance rapidly. The low-flow isoflurane anesthesia is safe, feasible and economical.
