哮喘患者血清转化生长因子β_1水平与脉冲振荡肺功能指标的相关性分析

摘　　要：目的探讨不同程度支气管哮喘(哮喘)患者的血清转化生长因子β1(TGFβ-1)变化,及其与脉冲振荡(IOS)技术测定呼吸阻抗指标间的关系。方法用酶联免疫吸附试验检测29例轻度、27例中重度哮喘患者的血清TGFβ-1;用IOS技术检测其呼吸阻抗指标呼吸总阻抗(Zrs)、总气道黏性阻力(R5)、中央气道黏性阻力(R20)、周边气道总黏性阻力(R5-R20)、周边弹性阻力(X5)及共振频率(Fres)。并设30例健康者作为对照组。结果哮喘组血清TGFβ-1与对照组比较、轻度与中重度哮喘患者比较,均有统计学差异(P<0.05,<0.01);哮喘组Zrs、Fres、R5、R20、R5-R20、X5负值均高于对照组(P<0.05,<0.01);血清TGFβ-1水平与Zrs、Fres、R5、R5-R20呈显著负相关,与X5呈显著正相关(P<0.01,<0.05)。结论哮喘患者血清TGFβ-1下降及呼吸阻抗升高与其病情严重程度有相关性,血清TGFβ-1水平能间接反映哮喘患者的气道阻力变化,作为哮喘诊断及病情判断的参考指标。Objective To investigate the change of serum TGF-β1 and the relationship between serum TGF-β1 and the main indices of breathing impedance by IOS in different pathogenetic condition of asthmatics. Methods TGF-β1 levels were detected by ELISA in 29 cases of mild asthmatics and 27 moderate-severe asthmatics. Respiratory impedance （ Zrs）, total airway resistance （ R5 ）, central airway resistance （ R20 ）, peripheral resistance （ RS-R20）, peripheral capacitance （XS） and resonance frequency （Fres） were measured by IOS. 30 cases of healthy persons were acted as control group. Results The serum TGF-β1 concentration revealed statistical difference between the asthmatics and the control group, and between the mild asthmatics and the moderate-severe asthmatics（P 〈 0.05, 〈 0.01 ）. The Zrs, Fres, R5, R20, R5-R20, X5 negative values in the asthma group were higher than that in the control group（ P 〈 0.05, 〈 0.01 ）. The serum TGF-β1 had negative correlation with Zrs, Fres, RS, R.5-R20, and had positive correlation with X5 （ P 〈 0.01, 〈 0.05 ）. Conclu- sions Low serum TGF-β1 level and high breathing impedance indexes have direct relation with the disease severity in .asthmatics, serum TGF-β1 level could indirectly reflect the changes of airway resistance in asthma patients, it could be used as a reference index for diagnosis and judgment of patient‘ condition with asthma.
