摘  要:目的调查川北地区鲍曼不动杆菌耐药性及β-内酰胺酶基因型。方法用Vitek 32细菌鉴定系统对细菌进行鉴定。用琼脂稀释法测定鲍曼不动杆菌对16种抗菌药物的最低抑菌浓度(MIC)。用聚合酶链反应(PCR)法检测β-内酰胺酶耐药基因,并对耐药基因测序分析。结果78株鲍曼不动杆菌对亚胺培南全部敏感,对美罗培南的耐药率为1.3%,对头孢哌酮/舒巴坦的耐药率为37.2%,对其它13种抗菌药的耐药率均在60%以上。21株多重耐药鲍曼不动杆菌均携带TEM型基因,其中,同时还携带SHV和CTX-M型基因的分别有14株和7株,经DNA测序,TEM基因均为TEM-128,CTX-M基因为CTX-M-2,SHV基因可能为SHV-5、SHV-1b、SHV-56、SHV-71或SHV-96。结论川北地区鲍曼不动杆菌对临床常用抗菌药存在严重耐药现象,β-内酰胺酶耐药基因型以TEM-128为主,同时也有SHV和CTX-M,部分菌株携带两种以上β-内酰胺酶耐药基因参与细菌耐药性的形成。Objective To investigate the resistance and genotyping of β-Lactamases in Acinetobacter baumannii isolated from North Sichuan area. Methods After Acinetobacter baumannii was identified by Vitek 32 system, the agar plate method was prefromed to detect MIC of 16 kinds of antimicrobial agents against Acinetobacter baumannii, and genotypes of of β-Lactamases were analyzed by PCR and verificated by DNA sequencing. Results All of 78 strains of Acinetobacter baumannii were sensitive to imipenem. The resistant rates of the isolates tested to meropenem and cefoperazone/sulbactam were 1.3% and 37.2%, respectively, but to other β antimierobial agents were more than 60%. Twenty-one strains of muhidrug resistant Acinetobacter baumannii were detected by PCR and DNA sequencing, 21 strains produced TEM-128 , in which, 7 strains produced CTX-M-2 and 14 strains produced SHV-5. Conclusion The resistance of Acinetobacter baumannii isolated from North Sichuan area was serious. Acinetobacter baumannii in the regions mainly carried TEM-128, and some also carried SHV and CTX-M. Parts of strains contained more than 2 types of β-lactamase genes.
