胎儿单脐动脉畸形妊娠中晚期的彩色多普勒超声诊断价值

摘　　要：目的对妊娠中晚期单脐动脉的声像特征及其合并胎儿其它系统发育畸形的分析,揭示彩色多普勒超声检查的意义。方法2004年3月～2008年5月间,我们用彩色多普勒超声常规检查胎儿脐带血管数目,发现单脐动脉32例,同时仔细检查胎儿其它器官,并追踪其临床情况、妊娠结局及病理检查。结果①单脐动脉畸形时声像图特征是脐带内正常三支脐血管螺旋状排列特征消失,只有一条动脉和一条静脉。②在32例单脐动脉畸形病中,单纯单脐动脉畸形21例,合并其他器官发育畸形11例,其中并心脏异常5例,肢体异常4例,1例消化系统异常,1例前腹壁异常。结论彩色多普勒超声是诊断胎儿单脐动脉畸形的可靠手段,并能进一步检查胎儿是否合并其他器官发育畸形,从而指导对妊娠中晚期孕妇的临床处理、优化人口素质有深远的意义。Objective Through analyzing the uhrasonographic features of single umbilical artery（SUA） and SUA associated fetal other malformations during mid and late - pregnancy, to reveal the significance of color Doppler ultrasound. Methods From March 2004 to May 2008, we routinely checked the number of fetal umbilical cord vessels, and found 32 SUA eases, at the same time we carefully examined fetal other organs and then followed up the clinical conditions and pregnancy outcome and pathological examination. Results （1）An umbilical cord had only two vessels in SUA, an artery and a vein while the normal spiral structures disappeared. （2）32 SUA cases were diagnosed by color Doppler ultrasound. SUA as an isolated finding occurred in 21 cases. 11 SUA had associated malformation, including 5 cardiovascular anomalies, 4 limbs anomalies, 1 digestive system anomaly, 1 anterior abdominal wall anomaly. Conclusion Color Doppler ultrasound is the reliable means to diagnose the fetal SUA, and to further examine whether the fetus associated the malformations of other organs. And to guidance on the clinical treatment of pregnant women during mid and late - pregnancy. It has far - reaching significance for optimization of the population quality.
