摘 要:目的观察胰岛素强化治疗前后初发2型糖尿病(T2DM)患者外周血单核细胞中核因子κB(NF-κB)表达水平的变化,探讨胰岛素的抗炎作用机判。方法健康成人20例为正常对照(NC)组,新诊断T2DM 20例为DM组,DM组胰岛素强化治疗2周。治疗前后采用ELISA、FCM法检测NF-κB表达水平,同时测定治疗前后WBC、N%和FIns,计算HOMA-IR。结果与NC组比,DM组NF-κB表达水平显著增高(P<0.01);DM组与治疗前比,治疗后NF-κB水平显著降低(P<0.05),WBC、N%和HOMA-IR也明显降低(P均<0.01)。结论 2型糖尿病患者体内存在低度炎症,胰岛素强化治疗可降低单核细胞NF-κB表达,减轻炎症反应。Objective To investigate the effect of intensive insuIin therapy on the expression level of nuclear factor-kB in peripheral blood mononuclear cell (PBMC) in newly-diagnosed type 2 diabetics. Methods Twenty normal individuals and 20 newly diagnosed type 2 diabetics were selected. All diabetic patients were given intensive insulin therapy for two weeks. NF-kB expression in peripheral blood mononuclear cell was measured by enzyme-linked immunosorbent assay (ELISA) and flow cytometry (FCM) before and after insulin treatment. The changes of FBG,while blood cell count (WBC), percent of neutrophilic granulocyte(N%) and fasting serum insulin(Fins) were assayed. Results The expression level of nuclear factor-kB detected by ELISA and FCM in type 2 diabetes patients was significantly higher than that in control subjects (P〈0.01). The expression level of nuclear factor-kB after treatment was significantly decreased (P〈0. 05), and the levels of WBC, N% and HOMA-IR were also decreased after insulin intensive therapy (all P 〈 0. 01 ). Conclusions There may be a low-degree of inflammation reaction in type 2 diabetics. Intensive insulin therapy can decrease the level of NF-kB in PBMC and inflammatory reaction.
