单纯性肥胖儿童血清瘦素与相关指标的关系研究

摘　　要：目的探讨单纯性肥胖儿童血清瘦素水平与其他内分泌激素及血脂异常的关系，并分析在儿童生长发育过程中的作用。方法以10～15岁单纯性肥胖和正常对照儿童各40例为研究对象，以放射免疫法、酶法测定血清中的瘦素、胰岛素、生长激素水平及血脂水平。结果肥胖组血清瘦素和胰岛素水平均显著高于对照组（P〈0．01），而生长激素水平显著低于对照组（P〈0．01），总胆固醇、三酰甘油、低密度脂蛋白显著高于对照组（P均〈0．01），高密度脂蛋白低于对照组，但无显著性差异（P〉0．05）。结论单纯性肥胖儿童存在高胰岛素血症，瘦素抵抗与生长激素分泌减少、血脂代谢紊乱密切相关，从而影响儿童的生长发育。Objective It is to discuss the relationship between serum Leptin level and other endocrine hormone and dyslipidemia of simple obesity children and to analyze their effect in the course of children＇s growth and development. Methods 40 cases of simple obesity children and 40 cases of normal children who were 10 - 15 years old were chosen and taken as investigative object. The levels of Leptin, insulin and somatotropin in serum and the level of blood fat were determined with radioimmunity method and enzymic method. Results The serum levels of Leptin and insulin in obesity group were significantly higher than those in control group （P〈 0.01）, yet the serum level of growth hormone in obesity group was significantly lower than those in control group （ P 〈 0.01 ）. The levels of total cholesterol, triacylglycerol and low density lipoprotein in obesity group were significantly higher than those in control group （P 〈 0.01 ）, the level of high density lipoprotein in obesity group was lower than those in control group but there was no significant difference （P 〉 0.05）. Conclusion There is hyperinsulinism in simple obesity children. Leptin resisting has close correlation with lower growth hormone and lipid metabolic disorder, which can influence children＇s growth and development.
