股骨转子间骨折DHS内固定术经肌间隙入路的优点——附43例病例分析

摘  要:目的:评价股骨转子间骨折DHS内固定手术中切断股外侧肌止点显露股骨大转子及股骨近端的优点。方法:对23例股骨转子间骨折采用切断股外侧肌止点,从股外侧肌、股二头肌肌间隙入路,显露术区的方式与20例采用切开股外侧肌显露术区的术式进行对照。比较两组手术切口长度、手术时间、手术中出血量、手术后引流量、手术前后肌酸激酶的变化、术后负重时间等参数,对比分析两组治疗效果。结果:手术时间、手术中出血量、手术后引流量及肌酸激酶变化参数有显著性差异。结论:切断股外侧肌止点方法比切开股外侧肌手术方法创伤小、出血少、显露好、操作简便、成功率高。Objective:To appraise the merits of operative approach with insertion of musculus vastus lateralis cut and Greater trochanter of femur as well as proximal end of femur revealed in DHS internal fixation for intertrochanteric fracture. Methods :23 cases of patients with intertrochanteric fracture were done with the style of insertion of musculus vastus lateralis cut, approach through spatium intermusculare of musculus vastus lateralis and biceps femoris and area for operation revealed, the other 20 cases were handled with the style of musculus vastus lateralis cut to reveal opera- tion zone. Then they were contrasted, and such items as lengths of Operative incision, times on operations, counts of blood loss in operations, rainage counts post-operation, changes on creatine kinase pre-and post-operation, times of weight loading post-operation were recorded and contrasted, their therapeutic efficacy were analyzed. Results: There had significant difference on times on operations, counts of blood loss in operations, rainage counts post-operation, and changed parameters on creatine kinase in two groups. Conclusion: The method of insertion of musculus vastus lateralis cut had such merits than that of musculus vastus lateralis cut as small wound, less blood loss, better revealing, convenient manipulation and higher achievement ratio.
