摘 要:目的:探讨急性等容血液稀释(ANH)在全髋关节置换术中临床应用价值。方法:将80例ASAIⅡ-级拟行全髋关节置换术患者随机分成ANH组和对照组,每组各40例,ANH组于麻醉后行血液稀释。两组分别于诱导麻醉后及术后1 d检测红细胞压积(HCT)、血清肌酐(Cr)浓度及血浆凝血酶原时间(PT),检测血液动力学参数以及术后并发症,并进行对比分析。结果:ANH组术后HCT、Cr、PT及血液动力学参数与对照组比较,差异均无统计学意义(P>0.05);而ANH组术中输注浓缩红细胞的患者数和术后并发症发生率显著低于对照组,差异均有统计学意义(P<0.05)。结论:ANH对全髋关节置换术患者生理功能影响不明显,但能显著减少异体输血及术后并发症的发生。Objective:To evaluate the clinical effect of acute normovolemic hemodilution in total hip arthroplasty. Methods :80 cases of patients who were planed to be treated with total hip arthroplasty were randomly divided into ANH group (40 cases) and control group (40 cases). Patients in ANH group were treated with hemodilution after anes- thesia. Hematocrit ( HCT), creatinine ( Cr), prothrombin time (FY) and parameters of hemodynamics of patients in both groups were detected after anesthesia and one day after operation. The postoperative complications were also ana- lysed. Results: There were no statistical difference in HCT, Cr, FF and parameters of hemodynamics between 2 groups(P 〉0.05) . The number of patients who were infused with concentrated red blood cells and the incidence of postoperative complications in ANH group were lower than those in control group ( P 〈 0.05 ). Conclusion : ANH has few influence to the physiological function of patients treated with total hip arthroplasty, but it can significantly decrease the incidence of blood transfusion and postoperative complications.
