摘 要:目的探讨测定心力衰竭(HF)患者血浆脑钠肽(BNP)、血清尿酸(UA)水平在临床应用中的意义。方法测定102例HF患者,按NYHA心功能分级分为:Ⅱ、Ⅲ、Ⅳ级及30例对照组血浆BNP、血清UA、左心房内径(LAd),左室舒张末期内径(LVEDd)及左室射血分数(LVEF)。结果HF患者UA增高组血浆BNP、LAd、LVEDd均显著高于UA正常组;LVEF显著低于UA正常组(P<0.05)。HF患者血浆BNP、血清UA均显著高于对照组(P<0.05),随HF程度的加重而显著增加。NYHA心功能分级Ⅲ、Ⅳ级患者LAd、LVEDd均显著高于Ⅱ级组;LVEF显著低于Ⅱ级组(P<0.05)。结论血浆BNP、血清UA对HF患者的诊断、治疗、病情和预后判断具有重要意义。Objective To study the role of plasma brain natriuretic peptide (BNP) and serum uric acid (UA) concentration in the clinical application to patients with heart failure (HF). Methods The plasma BNP, the serum UA, LAd, LVEDd and LVEF were measured in 102 patients with HF and 30 controls. Results HF patients with high UA bad higher plasma BNP, LAd and LVEDd than HF patients with normal UA; LVEF was lower than those with normal UA( P 〈 0. 05). The plasma BNP and the serum UA in patients with HF were significantly higher than those in control group( P 〈 0.05), and increased with severity of HF. LAd, LVEDd of NYHA functional class Ⅲ and Ⅳ were higher than those of NYHA functional class Ⅱ ; LVEF was lower than that of NYHA functional class Ⅱ (P 〈 0.05). Conclusion The level of plasma BNP and serum UA plays an important role in diagnosis, treatment, state of an illness and prognosis of patients with HF.
