摘　　要：目的探讨经胸膜外、腹膜后保留肋骨脊柱前路治疗胸、腰椎骨折及结核的临床疗效。方法2003年10月～2008年2月对65例胸、腰椎骨折及结核经胸膜外、腹膜后保留肋骨脊柱前路伤椎次全切,病灶清除,取髂骨椎间植骨融合钛钢板螺钉内固定术。结果术后随访2～50个月,1例并发肺栓塞死亡,64例椎间植骨牢固愈合,脊柱序列正常,钢板螺钉无松动。结论经胸膜外、腹膜后保留肋骨胸腰椎侧前方减压,植骨内固定是治疗椎体骨折及结核的理想方法之一。Objective To study the clilical efficacy of treating thoracic and lumbar fractures and tuberculosis through spinal anterior by retaining the ribs with extrapleural-retroperitoneal approach .Methods In 65 cases of thoracic and lumbar fractures and tuberculosis, retain the ribs with extrapleural-rctroperitoneal approach and partly cut the injury anterior spinal vertebra the focus cleared. Iliac bone graft fushion with titanium screw plate fixation. Results In the following 2 - 50 months, 1 cases of pulmonary embolism complicated by the death ,64 cases of intervertebral bone healing firm, the normal sequence of the spine, and no plate screws loose. Conclusion Retaining thoracic ribs with extrapleural-retroperitoneal approach, the thoracic and lumbar decompression and bone graft fixation is an ideal one of the ways to treat vertebral fractures and tuberculosis.
