超声心动图评价代谢综合征对高血压患者左室肥厚及功能的影响

摘　　要：目的：应用超声心动图探讨代谢综合征（MS）对高血压患者左室肥厚（LVH）及功能的影响。方法：选择63例原发性高血压患者，按NCEP—ATP提出的MS诊断标准（腹围按照亚太地区的标准）：分为高血压合并MS（HMS）组和单纯高血压（EH）组；另选健康体检者30例作为对照组。测定所有患者超声心动图的各种参数，计算左室质量指数（LVMI）和相对室壁厚度（RWT）。于心尖四腔、心尖两腔和心尖三腔切面获得组织速度成像（TVI）图像，应用变率成像（SRI）技术分析左室各节段心肌的应变率曲线，并计算左室收缩期、舒张早期、舒张晚期峰值应率平均值（mSRs、mSRe、mSRa）。结果：高血压患者与正常对照组比较：mSRs，mSRe及SRe／SRa显著低于对照组（P〈0．05），HMS组与EH组比较：LVH的比率明显增高（P〈0．01），而mSRs，mSRe及SRe／Sra明显降低（P〈0．05）。结论：MS与高血压患者的LVH密切相关，因此临床对LVH的干预，不仅要纠正血流动力学异常，还应对MS各因素积极干预，以有效地逆转LVH；应变率成像技术为定量、准确评价高血压患者左室心肌功能提供了新方法。Objective：To evaluate the influence of metabolic syndrome（MS） on the left ventricular hypertrophy and the left ventricular function innon-diabetic hypertensive patients using echocardiographic. Methods ：A total of 63 essential hypertensives included in which 32 patients were diagnosed as MS（HMS）, 31 patients with hypettension alone（EH）as controls. Another 30 normal volunteers accepted various examination. Left ventricular mass index （LVMI） and relative wall thickness （RWT） were determined by echocardiography. Systolic and diastolic strain rate were measured with SRI at two ,three and four chamber views. Mean Strain rate （mSR） and SRe/SRa were obtained. Results ..In all subjects ,mean SRs,mean SRe and SRe/SRa are significantly lower in HMs group and in EH group than normal group. Compared with hypertension alone（EH） patients,hypertensive patients with MS（HMS） were shown to have more prevalent of LVH ,and mSRe ,mSRa ,SRe/SRa showed statistic difference. Conclusion ：Metabolic syndrome is associated with left ventricular hypertrophy（LVH） in hypertension patients,Therefore,to prevent and reverse the progress of LVH effectively,it is necessary to not only reduces the abnormal high blood pressure,but also all the risk factors involved by MS is strongly advocated.
