臂丛神经阻滞治疗肩周炎的临床研究

摘　　要：目的探讨臂丛神经阻滞治疗肩周炎的效果。方法将我院收治109例肩周炎患者随机分为1组57例,2组52例,1组采用臂丛神经阻滞疗法;2组采用肩关节功能锻炼,并辅用药物治疗。2个疗程后观察两组患者的临床疗效。结果1组治愈49例(86.0%),好转8例(14.0%);2组治愈24例(46.2%),好转15例(28.8%),无效13例(25%)。两组患者疗效间差异有统计学意义(P<0.01)。结论臂丛神经阻滞治疗肩周炎是一种效果良好的方法。Objective To approach and evaluate the clinical efficacy of the brachiplex nerve block in treating the patients with the periarthritis of shoulder. Methods A total of 109 periarthritis of shoulder patients were randomly assigned into two groups. Group I （ n = 57） ： After had catched the different feel on the 57 patients through the interscalene approach pricking, they were injected with the mixed physic liquor that it was consisted of 2% Lidocaine 15ml, VitB1 100mg, VitB6 100mg, VitB12 lmg and Triamcinolone Acetonidel5mg weekly, four times is called one course of treatment. It was reqired to have been treated for two course of treatment; Group Ⅱ （n =52） ： 52 patients taked Brufen, 2 -3 pellets, bid, taked VitBx and VitB6 2 pellets, bid respectively. Articulatio capitis humeri functional exercise, qd. A month was called one course of treatment, for two months continuously. Results （Ⅰ） In the group Ⅰ： cure 49 cases （86.0%）, improving 8cases （14.0%）; In the group Ⅱ ： cure 24 cases （46. 2% ）, improving 15 cases （28.8%） ; （2） Comparing of the total cure rate of the group I with that of the group lI showed that there was a good statistically significant difference （ P 〈 0. 0 1 ）. Conclusion The efficacy of the brachiplex nerve block in treating the patients with the periarthritis of shoulder is a good method of treatment.
