摘 要:目的探讨吸入2.4%七氟醚(SEV)对感染性休克大鼠肺功能及存活率的影响。方法32只清洁级SD大鼠随机分为4组,每组8只:空白组(Sham组),单纯行开腹关腹手术;感染性休克组(CLP休克组),用盲肠结扎穿刺法建立感染性休克模型;SEV-Ⅰ组,CLP法建立感染性休克模型1 h后吸入2.4%SEV30 min;SEV-Ⅱ组,CLP法建立感染性休克模型3 h后吸入2.4%SEV30 min。各组均在感染性休克模型建立后1、3、5 h记录心率、平均动脉压(MAP),行动脉血气分析。在感染性休克模型建立5 h后处死大鼠,进行肺水含量、肺血管通透性(伊文氏蓝染色法)测定。另有40只大鼠分组方法同上,每组10只,观察CLP休克后12 h存活率。结果与Sham组相比,CLP休克组、SEV-Ⅰ组和SEV-Ⅱ组大鼠的HR明显增快,MAP明显下降,血气分析参数pH值、PaO2、PaCO2明显下降,肺水含量和肺血管通透性明显升高(均P<0.05);但3组间差异无统计学意义(P>0.05)。Sham组大鼠12 h存活率为100%,CLP休克组为0%,SEV-Ⅰ组为12%,SEV-Ⅱ组为0%,感染性休克组、SEV-Ⅰ组和SEV-Ⅱ组间差异无统计学意义(P>0.05)。结论感染性休克后吸入2.4%七氟醚30 min不能改善动物的肺功能和12 h死亡率。Objective To investigate the effects of sevoflurane on pulmonary function and 12 h survival rate in cecal ligation and puncture (CLP)-induced septic shock rats. Methods Thirty two Sprague-Dawley (SD) rats were randomly divided into four groups:sham group, septic shock group (CLP shock group), sevoflurane- Ⅰ (SEV- Ⅰ )group and sevoflurane-Ⅱ (SEV-Ⅱ )group. Rats in CLP shock group received cecal ligation and puncture to induce septic shock model, rats in SEV- Ⅰ group received 2.4% sevoflurane for 30 min one hour after CLP shock, rats in SEV- Ⅱ group received 2.4% sevoflurane for 30 min three hours after CLP shock. The changes of the heart rate ( HR ), mean arterial pressure ( MAP), arterial blood gas analysis were measured at 1,3,5 h after CLP shock in all groups. Lung wet/dry weight ratio (W/D) and pulmonary vascular permeability were measured 5 h after CLP shock in all groups. Second part of the experiment was conducted to observe 12 h survival rate after CLP shock in 40 rats,grouping method was same as the obove. Results MAP decreased and HR increased in CLP shock, SEV- Ⅰ and SEV-Ⅱ groups compared with the sham group (P 〈 0.05). Parameters from arterial blood gas analysis were decreased in CLP shock, SEV- Ⅰ and SEV- Ⅱ groups ( P 〈 0.05 ). Lung W/D and lung vascular permeability were higher in CLP shock, SEV- Ⅰ and SEV- Ⅱ groups than in the sham group ( P 〈 0.05 ). There were no significant difference of all above data between CLP shock, SEV-Ⅰ and SEV- Ⅱ groups (P 〉0.05). The 12 h survival rate was 0% ,12% ,0% in CLP shock,SEV-Ⅰ and SEV-Ⅱ groups respectively, there was no significant difference among them (P 〉 0.05 ). The 12 h survival rate in sham group was 100%. Conclusion Inhalation of 2.4% SEV for 30 min in CLP-induced septic shock could not improve the pulmonary function and 12 h survival rate in CLP shock rats.
