成人接种国产重组酵母乙肝疫苗的免疫效果分析

摘　　要：目的探讨国产重组酵母乙型肝炎疫苗对成人的免疫效果及其安全性。方法在某合资企业选择700例20~55岁的健康成人,随机分2组,均以0、1、6程序分5μg/0.5ml、10μg/1.0ml的乙肝疫苗,全程免疫后1个月观察免疫效果。结果①5μg/0.5ml组：抗-HBs阳转率为94.4%,男女之间无差异,〈35岁组显著高于≥35岁组;抗-HBs滴度的中位数为365.90mIu/ml,男女之间无差异,〈35岁组显著高于≥35岁组;②10μg/1.0ml组：抗-HBs阳转率为94.6%,男女之间、〈35岁组和≥35岁组之间无差异;抗-HBs滴度的中位数为455.55mIu/ml,男女之间无差异,〈35岁组显著高于≥35岁组;③两组比较：两组免疫后的感染模式无差异,抗-HBs阳转率无差异,两组免疫后抗-HBs滴度无显著差异。结论国产重组酵母乙型肝炎疫苗对成人具有良好的免疫原性和安全性,35岁以下成人接种疫苗效果优于35岁以上,接种疫苗后抗-HBs持续保护时间有待于进一步观察。Objective To observe and evaluate the safety and immunogenicity of recombinant yeast- derived hepatitis B vaccine after inoculating into adults. Methods 700 adults aged 20-55 years old, selected from the same enterprise, were random divided in two groups. 5μg/0.5ml and 10μg/0.5ml YDV were given to each group three times, and to investigate the effect one month after the third injection. Results （1）5μg/0.5ml group： Positive rate of anti-HBs was 94.4%, there was no significant difference between male and female, but in more than 35y group the rate was higher than younger group. The average antibody titer was 365.90mlu/ml, without difference in sex, higher titer in less than 35y group. （2）10μg/0.5ml group： Positive rate of anti-HBs was 94.6%, no difference was found both between each sex and age group. The average antibody titer was 455.55mIu/ml, without difference in sex, but in less than 35y group the antibody titer was much higher. （3）The infection mode and positive rate of two dose group displayed no significant difference. There was no significant difference between the average antibody level of the two groups. Conclusion The homemade recombinant yeast- derived hepatitis B vaccine showed satisfied safety and immunogenicity effect to adults.That 〈35 years old was considered better than those t〉 35 years old. But how long it could sustain needs to be further studied.
