摘　　要：目的:运用单光子发射型计算机断层显像(SPECT)技术,观察头针对帕金森病(PD)患者不同脑区血流状况的影响。方法:30例PD患者随机分为2组,针刺+药物组和药物组各半,分别接受5周的头针配合美多巴治疗、单纯美多巴治疗。治疗前后进行帕金森病统一等级量表(UPDRS)量表测评。从上述2组各随机抽取5例,治疗前后接受SPECT检测。结果:针刺+药物组和药物组患者在震颤、强直、运动障碍等方面均有明显改善,针刺+药物组的改善优于药物组(P<0.05)。10例PD患者病轻侧和病重侧rCBF比较,没有显著差异;药物组治疗后,两侧脑区局部脑血流(rCBF)均无明显改变;针刺+药物组治疗后,病重侧额叶、枕叶、基底节区、小脑rCBF均有明显升高(P<0.05),但病轻侧各脑区rCBF没有显著改变。结论:头针配合美多巴能改善患者的震颤、强直、运动障碍,其治疗PD的机制,与其改善脑区局部血流量有关。Objective： To study the effects of scalp acupuncture by single photon emission computer tomography （SPECT） on treating Parkinson＇s disease （PD）. Method： Thirty PD patients were divided into 2 groups randomly. Fifteen cases in treatment group were treated by scalp acupuncture combined with taking madopa. Another 15 cases in control group just received the treatment of madopa. All the patients received UPDRS assessment before and after the treatment. Then, five patients were chosen randomly from the above 2 groups and received SPECT examinations before and after the respective treatment for 5 weeks. Results： The scores of UPDRS decreased significantly in both 2 group, but the decrease was more obviously than that of the control group （P〈0.05）. The values of rCBF between bilateral brain sides in 10 PD patients had not any obvious differences. After the treatment of madopa, rCBF of PD patients in the bilateral ROIs had not any significant changes. After the treatment of scalp acupuncture plus madopa, the rCBF in frontal lobe, occipital lobe, basic ganglion and cerebellum increased significantly in the severe-diseased side（P〈0.05）. While the rCBF in each ROI of the light-diseased side had not any significant changes. Conclusion： Scalp acupuncture, combined with madopa, could improve the rigidity, tremor, dyskinesia and rCBF in PD patients, which had close relationship with its effect on cerebral blood flow.
