无创正压通气治疗COPD合并呼吸衰竭的疗效观察

摘　　要：目的探讨无创正压通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的疗效。方法观察42例经过常规治疗效果不佳的COPD合并Ⅱ型呼吸衰竭患者应用NIPPV治疗前、24h后临床疗效及血气变化。结果所有患者经NIPPV治疗后病情明显好转,动脉血pH值恢复正常,PaO2和PaCO2分别高于和低于治疗前水平(P<0.01)。结论NIPPV对于COPD合并呼吸衰竭是一种切实有效的治疗措施。Objective To evaluate the effect of noninvasive positive-pressure ventilation（NIPPV） in the treatment of patients with chronic obstruction lung disease （COPD）combining type Ⅱ respiratory failure.Methods The clinical therapeutic effect was observed and blood gas was analyzed before and 24h after NIPPV for 42 patients with COPD plus type Ⅱ respiratory failure who were not susceptible to routine therapy.Results All patients resumed gradually and felt better and their artery blood pH,partial pressure of O2 and CO2 nearly returned to normal after treatment （P〈0.01）.Conclusion NIPPV is an effect way for treating COPD combining with respiratory failure.
