摘 要:目的:探讨2型糖尿病肾病患者尿微量白蛋白(mA1b)与脂质代谢和糖化血红蛋白(GHbA1c)异常间的关系。方法:72例2型糖尿病肾病患者根据尿微量白蛋白排泄率分为正常白蛋白尿(NAU)组、微量白蛋白尿(MAU)组和临床蛋白尿(CAU)组,另选30例健康体检者作为对照组(NC)组。检测各组对象的空腹血糖(FBG)、糖化血红蛋白(GHbA1c)与血脂指标。结果:T2DM组FBG、GH-bA1c、甘油三酯(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)的水平显著高于NC组(P<0.01),而高密度脂蛋白胆固醇(HDL-C)明显低于NC组(P<0.05)。CAU组、MAU组与NAU组比较,结果显示FBG、GHbA1c、TG、TC、LDL-C的水平显著升高(P<0.01),而HDL-C明显降低(P<0.05)。结论:血糖控制水平、糖化血红蛋白及高脂血症与糖尿病肾病尿微量白蛋白关系密切,各项指标的联合检测对2型糖尿病肾病的诊断与评价有重要意义。Objective:To investigate the relationship between glycosylated hemoglobin (GHbAlc),the disturbance of lipids metabolism and urinous albumin (mAlb) excretion rate in type 2 diabetic nephropathy(DN). Methods:To choose 72 eases with DN and 30 healthy persons (NC).Aecording to urinous albumin excretion rate,the patients were classified into three groups:normal albuminuria (NAU),microalbuminufia (MAU)and clinical albuminuria(CAU). To detect FBG,HhAle,TC,TG,LDL-C and HDL-C. The 72 cases were divided into 3 groups according to UAER.To observe the relationship between GHbAle,the disturbance of lipids metabolism and urinous albumin excretion rate.Results:FBG,GHbAlc,TG,TC and LDL-C were increased along with UAER.And HDL-C was decreased along with UAER. The difference had statistical significance between DM group and NC group(P〈0.05) for all indexes. The difference also had statistical significance between CAU group,MAU group and NAU group (P〈0.05).Conelusion:There is close relationship between the blood glueose,GHbAlc ,the lipids metabolism and UAER.They are important in diagnosising and evaluating type 2 diabetic nephropathy.
