全椎板切除椎管减压术治疗胸椎黄韧带骨化症29例围术期护理

摘　　要：目的:探讨全椎板切除椎管减压术治疗胸椎黄韧带骨化症的护理方法。方法:对29例胸椎黄韧带骨化症患者有针对性地做好术前准备、心理护理、症状护理、适应性训练,术后严密观察双下肢感觉运动变化、切口引流情况,预防下肢深静脉血栓形成、脑脊液漏等并发症,并进行康复训练指导。结果:本组29例术后症状立即缓解,2例经8 d与10 d加重期后逐渐缓解,1例并发脑脊液漏,经治疗后痊愈。结论:全椎板切除椎管减压术治疗胸椎黄韧带骨化症难度大,围术期实施有效的护理干预是提高手术成功率和降低并发症发生率的重要保证。Objective ： To explore nursing methods in the treatment of patients with thoracic ossification of ligamentum flavum through vertebral canal decompression by total lamineetomy. Methods： Specific preoperative preparation, psychologi- cal care, symptom care and adaptive training were applied to 29 patients. Close observation of the changes of kinaesthetic sense in lower limbs, drainage, prevention of deep vein thrombosis and complications such as cerebrospinal fluid leakage and the guidance of rehabilitation training were carried out. Results ： The postoperative symptoms of 29 patients immediately relieved, and the symptoms of 2 patients gradually eased when it had got worse for 8 - 10 days; one patient got complicated cerebrospinal fluid leakage and fully recovered after treatment. Conclusion： The effective nursing intervention in periopera- tive period can markedly increase successful rate of the operation and reduce the incidence of complications because of the difficulty of total laminectomy in the treatment of thoracic ossification of ligamentum flavum.
