摘　　要：目的研究血管迷走性晕厥(VVS)患者在直立倾斜试验(HUTT)中不同反应类型的分布特征,探讨HUTT不同反应类型与VVS临床表现的关系。方法收集2007年1月至2008年4月在中南大学湘雅二医院就诊的HUTT阳性的VVS患者90例,分析HUTT不同反应类型的临床特点,比较各反应类型在性别、发病初始年龄及HUTT反应时间等方面的差异。结果90例HUTT阳性患者中男29例,女61例;血管抑制型68.9%(62/90),混合型30.0%(27/90),心脏抑制型1.1%(1/90)。VVS患者发病季节以夏季多见(47.8%)。各反应类型之间男女性别比例差异无统计学意义(P>0.05);混合型患者病程显著长于血管抑制型患者(P<0.05);年龄、发病初始年龄、基础心率及基础收缩压和舒张压,在血管抑制型和混合型之间比较差异无统计学意义(P>0.05);基础直立倾斜试验(BHUT)阶段混合型的平均反应时间较血管抑制型缩短(P<0.05)。结论VVS以女性常见,VVS患者初发年龄主要集中在10岁左右,HUTT不同反应类型与临床表现有一定关系。Objective To investigate the different response types during the course of head-up tilt tests （HUTF） in patients with vasovagal syncope （VVS） and to explore the association of clinical features with different response types in HUTT in patients with VVS. Methods From January 2007 to April 2008,90 patients who had a positive head-up tilt test （HUTT） result and the diagnosis of VVS were enrolled in this study,and clinical features of different response types in HUTT were analyzed and compared in different gender,the initial age of onset and response time of HUTT. Results Among 90 HUTT-positive patients （ 29 males, 61 females）, 68.9% were vasodepressor response, 30. 0% were mixed response and 1.1% （1/90） were cardioinhibitory response. Occurrence of VVS was mostly in summer（47.8% ）. There was no significant differences in sex ratio （ P 〉 0. 05 ） ; the course of the patients with mixed response was significant longer than that with vasodepressor response （P 〈 0.05 ） ;there were no statistical differences in the age, the initial age of onset, baseline heart rate, and baseline blood pressure between the patients with vasodepressor response and mixed response （ P 〉 0. 05 ） ; the mean time of vasogal episode of mixed response was shorter than that of vasodepressor response in BHUT stage（P 〈 0. 05 ）. Conclusion VVS was much more frequent in female. The peak age of this disease is around 10-yearold, and there was some association between different response types in HUTT and clinical features.
