络脉舒通颗粒剂治疗冠心病快速性心律失常的临床研究

摘　　要：目的观察络脉舒通颗粒剂治疗冠心病快速性心律失常的临床疗效,并初步探讨其作用机理。方法随机选择冠心病快速性心律失常72例,分为治疗组39例,对照组33例。治疗组给予络脉舒通颗粒剂治疗,对照组给予口服盐酸胺碘酮治疗。治疗4周后,观察治疗前后临床症状、动态心电图、心率变异性、心电图ST-T改变、P波离散度、QT离散度、化验室检查指标等,并进行统计学分析。结果(1)治疗组抗心律失常总有效率82.05%,与对照组无显著性差异。(2)在改善中医症状方面,治疗组明显优于对照组。(3)治疗组比对照组明显减小房性心率失常的P波离散度(Pd)。(4)在改善心率变异性(HRV)方面,治疗组明显优于对照组。(5)治疗组可降低血总胆固醇、甘油三酯、低密度脂蛋白水平。结论络脉舒通颗粒剂是临床治疗冠心病快速性心律失常的一个安全、有效的中药制剂,其组方体现清火化痰安神、祛风活血通络的治疗原则,其作用机理可能与改善缺血、调节血酯、改善心率变异性等方面有关。值得进一步深入研究。Objective To observe the clinical effect of Loumaishutong Granule on Tachyarrhythmia of Coronary Heart Disease and explore its mechanism. Methods 72 patients of Tachyarrhythmia of Cororeary Heart Disease were divided randomly into treatment group （39 cases） and control group （33 cases）. The treatment group was treated with Luomaishutong Granule. The controlgroup was treated with Amiodarone Hydrocholoride Tablets. The changes of clinical symptoms, Dynamic Electrocardiograthy and laboratory indexes were observed before and after treatment in both groups. Results （ 1 ） The effect rate of the treatment group in treating Tachyarrhythmia of Coronary Heart Disease is 82. 05%, which had no obvious difference to that of control group. （2） The treatment group is better than the control group in ameliorating clinical symptom. （3） The treatment group is superior to the control group in decreasing Pd of atrial Tachyarrhythmia of Coronary Heart Disease. （4） The treatment group is superior to the control group in improving HRV. （5）The treatment group can decrease CH,TG,HDL-C. Conclusion The methods of removing heat-phlegm and blood stasis restoring collateral calming the endopathic wind and palpitation is an effective therapeutic method, and Luomaishutong Granule is effective and safe for the patients with Tachyarrhythmia of Coronary Heart Disease.
