米非司酮配伍甲基睾丸素治疗子宫内膜增生疗效分析

摘　　要：目的探讨米非司酮配伍甲基睾丸素治疗子宫内膜简单型增生的临床效果。方法将94例40～54岁诊刮病理为子宫内膜简单型增生患者,给予每天米非司酮25mg+甲基睾丸素5mg,口服连续3个月,分别于治疗前后B超检查子宫内膜厚度,检测血清激素激素水平,刮宫组织病理检查,实验室检查血常规、肝功能,随访月经恢复情况及治疗效果。结果随访的92例用药后子宫内膜厚度减少(P<0.01)。用药后血清LH、T水平显著升高,FSH、P、E2显著降低(P<0.05～0.01),PRL无明显改变。随访92例,治疗后病理检查子宫内膜30例腺上皮呈增生早期改变,36例子宫内膜混合性改变,26例子宫内膜为分泌期改变,贫血患者全部纠正,2例肝功能轻度受损,治疗期间所有患者全部闭经,停药后9～56d月经恢复,6例进入绝经期,其余86例恢复月经,其中30例月经减少。结论米非司酮配伍甲基睾丸素能有效抑制子宫内膜增生,治疗效果满意。Objective To evaluate the effect of mifepristone combined with methyhestosterone on simple endometrial hyperplasia. Methods 93 cases of simple endometrial hyperplasia in age of 40-54 years diagnosed by histopathology were treated with 25mg mifepristone and 5mg methyhestosterone daily for 3 months. Endometrial thickness by ultrasound, pathology of endometriurn, sex hormones, hemoglobin and hepatic function were observed before and after treatment. Results According to ultrasound, endometrial thickness was reduced significantly from （ 8.03± 1.67 ） mm to （ 3.43 ± 0.85 ） mm（ P 〈 0.01 ） in 94 cases. The levels of LH and T were increased, and FSH, P and E2 were decreased （ P 〈 0.05 ± 0.01 ） after treatment. Histomorphological evaluation showed proliferative phase endometrium in 30 cases, and mixed change of endometrium in 36 cases. The endometrium of 26 cases was secretory phase. Hemoglobin of all patients recovered and hepatic function was damaged in 2 cases. Amenorrhea occurred in all patients during treating with mifepristone and methyhestosterone, but menstruation recovered in 9-56 days. Meno- pause happened in 6 cases. 86 cases recovered menstruation,including 30 cases whose menstruation volume was less. Conclusion Mifepristone combined with methyhestosterone can reverse the endometrial hyperplasia effectively and the curative effect is satisfactory.
