摘  要:目的研究心脏不停跳手术及停跳手术对肾素-血管紧张素-醛固酮系统(RAAS)影响的差异。方法将60例风湿性二尖瓣膜病变患者,随机分成实验组(不停跳组)和对照组(停跳组),其中实验组40例,对照组20例,分别于10个时点抽取静脉血,采用放射免疫分析法检测肾素(PRA)、血管紧张素Ⅱ(AngⅡ)和醛固酮(Aldo)水平。结果两组在CPB过程中PRA、AngⅡ和Aldo均有不同程度的升高;对照组升高更明显。结论体外循环心脏不停跳心内手术对心脏RAAS的影响可能较停跳心内手术小。Objective To explore the different effects on renin-angiotensin-aldosterone system (RAAS) of beating heart surgery and cardioplegia surgery during cardiopulmonary bypass (CPB). Methods Sixty patients suffering from rheumatic mitral disease were randomly divided into two groups:the trial group (beating heart surgery,40 cases)and the control group (cardioplegia surgery,20 cases). Venous blood samples were collected at ten different time intervals to examine the levels of plasma renin activity (PRA), angiotensin Ⅱ (Ang Ⅱ ) and aldosterone (Aldo). Result PRA, Ang Ⅱ and Aldo in two groups increased in varying degrees during CPB. PRA, Ang Ⅱ and Aldo in control group increased more significantly. Conclusion The results imply that the effect on coronary sinus RAAS of beating heart surgery during CPB is less than that in cardioplegia surgery.
