上皮钙黏素和β-连接素在儿童肝细胞癌中的表达及其临床意义

摘　　要：目的探讨上皮钙黏素（E-cad）和β-连接素（β-cat）在儿童肝细胞癌中的表达及其与肿瘤病理特征和预后的关系。方法用免疫组化Envision法检测42例儿童肝细胞癌组织中E-cad和β-cat蛋白的表达情况，并分析其与临床病理指标及预后的关系。结果（1）42例患儿中，E-cad异常表达者30例，占71．4％；β-cat异常表达者28例，占66．7％。（2）儿童肝细胞癌直径〉5cm、低分化、无肿瘤包膜及有复发转移的患儿组的E—cad和β-cat异常表达率显著高于儿童肝细胞癌直径〈5cm、高分化、有肿瘤包膜及无复发转移的患儿组（P〈0．05）。（3）E—cad和β-cat正常表达组的生存期和累积生存率显著高于异常表达组（P〈0．05）。结论E-cad和β-cat在儿童肝细胞癌有较高的异常表达率，它们的异常表达与肿瘤细胞的分化程度和侵袭、转移能力有关，异常表达者肿瘤容易发生复发、转移，预后较差。Objective To investigate the expression of E-caherin（E-cad）and β-catenin（β-cat） in children with hepatocellular carcinoma（HCC）and its relation to clinicopathological characteristics of the tumor and prognosis. Methods The expression of E-cad and β-cat was detected in 42 HCCs by immunohistochemical technique. The relation of the expression of E-cad and β-cat to the clinicopathological data and survival rates of children with HCC were evaluated. Results 1）E cad was aberrantly expressed in 30 patients（71.4 % ）and B-cat in 28 patients（66.7% ）. 2）The rates of aberrant expression of E-cad and β-cat were significantly higher in children with HCC of a tumor size larger than 5cm in diameter, poor differentiation and without tumor capsule than those of a tumor size of less than 5cm in diameter, well differentiation and with tumor capsule. The rates of aberrant expression of E-cad and β-cat were also significantly higher in children with recurrence or metastasis of HCC than those without （P〈0.05）. 3）The median survival time and the cumulative survival rates of children with HCC having proper E-cad and β-cat expression was significantly higher than that in those with aberrant expression （P〈0.05）. Conclusion The rates of aberrant expression of both E-cad and β-cat are high in children with HCC and they are correlated with tumor differentiation and the edibility of invasion and metasta- sis. Aberrant expression of E-cad and β-cat may indicate a high potential of recurrence or metastasis and poor prognosis.
