血液分析仪新鲜全血比对方案的建立及应用

摘　　要：目的建立一套规范的不同血液分析仪及同一血液分析仪不同检测模式的新鲜全血比对方案,保证临床标本在不同型号或同一仪器的不同检测(工作)模式下测定的结果一致性。方法半年期间每月定期对我室XE-2100A、XE-2100B、SE-9500、K-4500A和K-4500B共5台仪器的不同模式进行10份新鲜全血的检测,以XE-2100A开盖模式测定的值作为标准,其他仪器的测定值与其进行比对,分别计算差异百分率和差值。结果XE-2100A、XE-2100B、K-4500A和K-4500B比对结果较好,仅有XE-2100B的红细胞压积(HCT)超过允许范围2次,其余参数均在允许范围内,SE-9500的结果较差,共有8次超过允许范围,其中红细胞(RBC)有3次,HCT有5次。结论使用10份新鲜全血每月进行不同血液分析仪的检测比对是行之有效的比对方案,比对方案的实施对提高实验室检测质量有较大的临床意义。Objective To establish a standard comparison program for comparing the test results of one sample detected by different test modes of the same analyzer or different types of analyzers to make those results consistent. Methods A batch of 10 fresh complete blood samples were tested monthly on different tyes 5 Sysmex analyzers including XE-210OA,XE-210OB, SE-9500, K-4500A and K-4500B for half a year. Resuhs of open mode in Sysmex XE- 2100A were designated as the target values, and results of other types were compared. Percentages of differences or difference values of results were calculated. Results The comparison results of XE-2100A, XE-2100B, K-4500A and K-4500B were good, and only the result of haematocrit（HCT） of XE-2100B exceeded assigned range twice. The comparison results of SE-9500 were not good. Results of red blood cell（RBC） exceeded assigned range 3 times, while results of HCT exceeded assigned range 5 times. Conclusions It is an effective comparison program with 10 fresh complete blood samples monitoring different automated hematology counts monthly, which can improve test quality of complete blood cells.
