从痰瘀论治消渴初探

摘　　要：糖尿病属中医"消渴"范畴。其病因病机偏重于燥热偏盛,阴津亏耗;一般认为以阴盛为本,燥热为标,两者往往互为因果,燥热越盛则阴津越虚,阴津虚则燥热越盛。病变的脏腑主要在于肺、胃、肾。临床可见口渴喜饮、小便频数、浑浊而甜,消谷善饥、大便干结、疲乏消瘦。消渴病迁延日久,气阴两伤,痰浊瘀血痹阻脉络,可导致其他脏腑的多种病变。若目络瘀阻,血不循经或精不上承于目,则视物模糊、眼底出血,甚至目盲失明;若痰浊瘀血痹阻四肢脉络,则肢体麻痛,甚至肢端坏疽;若皮肤失养,脉络郁阻,兼感受风湿毒邪,则可出现皮肤瘙痒、溃疡等多种病变。治法为化痰活血,逐瘀通络,方用黄连温胆汤加减。药用黄连、竹茹、胆南星、枳实等。Diabetes mellitus belongs to the category of ＂xiaoke＂ （wasting thirst） in traditional Chinese medicine. It is due to preponderant blazing of dry heat and deficiency or wasting of yin fluid. Commonly, yin deficiency is considered to be the substance while dry heat is the external phenomenon. The two ofen interact both as the cause and effect. The more the dry heat rises, the more deficient will the yin fluid become, and vice versa. The main affected organs are the hmg, stomach and kidney. Will cause thirst and fond of drinking, frequent micturition, sweet urine, limosis and constipation, fatigue and emaciation. In lingering diabetes, when both yin and yang are damaged, the obstruction of the meridians and collaterals by phlegm turbidity and stagant blood can cause various pathological changes in other zangfu organs. If the meridian and collaterals of the eyes are obstructed, and blood is unable to circulate through the meridians or essence is unable to support the eyes upward, there will be blurred vision, eyeground bleeding and even blindness. If phlegm turbidity and stagnant blood are accumulated in the meridians and collatersls of the four extremities, there will be numbness and pain in the limbs, and even struction of the skin, if invaded by pathogenic wind, dampness and heat toxin, there will be skin pruritus, ulcreation and other pathological changes. The therapeutic method is activate blood folw and remove stasis, dissolve phlegm and clear the meridians, select modified Huanglian Wendan Tang （decoction）with such medicines as Huangliang, Zhuru, Dannanxing, Zhishi, etc.
