143例系统性红斑狼疮的自身抗体和常规检查分析

摘　　要：系统性红斑狼疮（SLE）是一种常见的自身免疫性疾病．研究探讨自身抗体及常规实验室检查对SLE的诊断及临床意义．间接免疫荧光法检测ANA抗体和抗ds-DNA抗体，斑点法检测抗ENA抗体，透射比浊法检测免疫蛋白，生化检查方法按试剂说明．143例SLE患者中，女性121例，ANA、dS-DNA、SS-A、n-RNP／Sm、Sm、SS-B阳性率分别为97．6％、57．8％、52．4％、48．9％、21．5％、11．7％，未检出Scl-70和Jo-1抗体．SLE患者血清抗ds-DNA抗体与抗Sm阳性有显著差别（P〈0．05）．在143例SLE患者中，IgG（35．66％）、IgA（36．36％）、RF（29．37％）升高，C3（85．3％）、C4（68．5％）降低．常规检查中，血红蛋白量（71．33％）、血钾（34．27％）、ALB（89．51％）降低，尿潜血和尿蛋白阳性率分别为28．67％和41．96％，ESR（83．22％）加快，肝功酶谱（30％-40％）升高．自身抗体检测结合常规检查有利于SLE的诊断和治疗．The systemic lupus erythematosus （SLE） is a frequent autoimmune disease. This study is to probe the diagnosis of SLE and the clinical significance of autoantibody test combined with routine exami- nations. Using indirect immunofluorescence to detect ANA and anti-dsDNA antibody, using the spotting method to detect anti-ENA antibody. Immunoprotein is detected by turbidimetry, biochemical examination is carried out according to the descriptions of the reagent. Of 143 SLE patients, 121 females were tested. whose positive ratio of ANA,ds-DNA,SS-A, n-RNP/Sm,Sm,SS-B were 97.6% ,57.8% ,52.4% ,48. 9%, 21.5%, 11. 7% respectively, hut their anti-Scl-70 antibody and anti-Jo-1 antibody were not discovered. There was a significant difference between the positive ratio of anti-ds DNA antibody and anti-Sin antibody in serum of SLE patients. Of 143 SLE patients,35.66% IgG,36. 36% IgA and 29.37% rheumatoid factor （RF） rose while 85.3% C3 and 68.5% C4 decreased. In routine examinations,71.33% heamoglobin （Hb）, 34.27% potassium and 89.51% albumin （ALB） decreased,the positive ratio of urine occult blood and that of urine protein were 28.67 % and 41.96 % respectively, and 83.22 % erythrocyte sedimentation rate （ESR） and 30% -40% zymogram of hepatic function increased. Autoantibody test combined with routine exami-nation is beneficial for the diagnosis and treatment of SLE.
