戈尔(Gore)补片修补治疗成人腹股沟疝、腹壁切口疝185例临床疗效分析

摘　　要：目的探讨戈尔(Gore)补片无张力修补术治疗成人腹外疝的疗效。方法回顾分析2003年1月至2007年6月我院普通外科应用美国戈尔公司生产的戈尔平片修补185例成人腹股沟疝及腹壁切口疝。单侧腹股沟斜疝121例,单侧腹股沟直疝27例,双侧腹股沟直疝22例,腹壁切口疝15例。其中复发疝35例,原发性疝150例。巨大斜疝50例,巨大直疝6例,均为单例。复发巨大斜疝13例,复发巨大直疝2例。结果185例中,无阴囊血肿,无睾丸缺血坏死,无下肢血栓形成,无明显异物感、疼痛感,无切口感染,随访1～3年,无复发。结论戈尔(Gore)疝补片无张力修补术治疗成人腹股沟疝及腹壁切口疝,复发率低,疼痛感、异物感轻,无切口感染,适合于各种腹股沟疝及腹壁切口疝的手术治疗,安全可靠。Objective To probe into the clinical effects of tension-free hernia repair by Gore hernia patch in abdominal external hernia. Methods An retrospective study was conducted to analysis the clinical effects of 185 adult patients with inguinal hernia or abdominal incisional hernia in our hospital between Jan, 2003 and june, 2007, who were treated by tension-free hernia repair using Gore hernia patch. Among the patients, 121 cases were unilateral indirect inguinal hernia, 27 cases unilateral direct inguinal hernia, 22 cases bilateral direct inguinal hernia and 15 cases ventral incisional hernia. There were 35 cases were relapsed hernia and others were primarily. There were 50 cases of tremendous unilateral indirect hernia including 13 relapsed eases, 6 cases of tremendous unilateral direct hernia including 2 relapsed cases. Results Among the 185 cases, there were no patients withs hematoma of scrotum, testicleischemic necrosis or thrombosis of the lower limbs, no patients with foreign body sensation, chronic pain or infection of the incisional wound. There were no relapsed case after 1 to 3 years follow-up. Conclusions Tension-free hernia repair by Gore hernia patch is a safe and reliable method to cure adult patients with inguinal hernia or abdominal incisional hernia with low recurrence rate, mild pain and foreign body sensation and no infection of the incisional wound, so it deserve to be popularized in clinical work.
