慢性肺心病急发期的植物神经类型与内分泌关系的研究

摘　　要：为研究慢性肺心病急发期植物神经类型与内分泌激素的关系，我们用放免法测定了８４例慢性肺心病急发期病人的Ｔ３、Ｔ４、ＴＳＨ、ＡＣＴＨ、皮质醇、ｃＡＭＰ、ｃＧＭＰ。从临床及实验室角度判定肺心病急发期植物神经类型，并分析与内分泌激素及预后之间的关系。结果表明，慢性肺心病急发期植物神经类型分为均衡型４８／８４例（５７．１％），交亢型１７／８４例（２０．２％），亢奋型１２／８４例（１４．３％），迷亢型７／８４例（８．３％）。各型与内分泌激素及预后关系为：交亢型Ｔ３、Ｔ４最高，迷亢型Ｔ３、Ｔ４、ＴＳＨ及皮质醇最低。均衡型病死率最低（６．３％），亢奋型第二、交亢型第三，迷亢型病死率最高（５７．１％）。提示不同植物神经类型的内分泌功能不同，慢性肺心病急发期的植物神经类型与甲状腺激素及皮质醇共同影响临床表现及预后。To study the relationship between plant nerve types and endocrine of acute stage of chronic cor pulmonale,we measured T_3,T_4,TSH,ACTH,cortisol,cAMP,cGMP in 84 patients with chronic cor pulmonale by radio immunoassay during their acute stage.The results showed that the plant nerve types of chronic cor pulmonal during acute stage were even type 48/84 (57.1%),sympathetic exciting type 17/84 (20.2%),exciting type 12/84 (14.3%) and parasympathetic exciting type 7/84 (8.3%).The relationships among all the types and hormone and prognosis were below:T3,T4,in sympathetic was the highest,T3,T4,TSH and cortisol in parasympathetic was the lowest.The mortality in even typy was the lowest (6.3%),exciting was the second,sympathetic was the third,parasympathetic was the highest (57.1%).The results indicated that the type of plant nerve,thyroid hormone and cortisol of acute stage of chronic cor pulmonal could affect the clinical manifestation and prognosis.The type of plant nerve was different,the function of endocrine was different.
