摘 要:目的探讨血浆溶血磷脂酸(LPA)水平与冠状动脉(冠脉)病变的相关性。方法检测344例疑诊为冠状动脉疾病(CAD)患者血浆LPA,并行冠脉造影,以病变支数及评分评价冠脉病变程度并分析与LPA水平之间的关系。结果无CAD者(冠脉评分=0分)与CAD者LPA水平差异有显著性[(1.27±0.36)μmol/L vs(1.89±0.90)μmol/L](u=9.01,P<0.05);不同冠脉病变支数、冠脉评分与LPA水平间有差异(F=9.69、F=53.37,P<0.05);在排除影响因素后,经多元逐步回归分析显示LPA是冠脉病变的危险因子。结论血浆LPA水平升高可以反映冠脉病变严重程度。Objective To evaluate the association of plasma lysophosphatidic acid(LPA) levels and the severity of coronary artery disease. Methods 344 subjects with suspected coronary artery disease(CAD) were enrolled in this study who were divided into non-CAD group and CAD group according to results of coronary angiography(CAG),the latter was further divided into subgroups based on the sum of diseased coronary branches.Blood samples for measurement of LPA level were taken before CAG.Patients with CAD were assigned to one of four subgroups according to LPA level with 25,50 and 75 percentile as cut-off points. Result In contrast to non-CAD group,plasma LPA level was obviously higher in CAD group[(1.27±0.36)μmol/L vs(1.89±0.90)μmol/L](u=9.01,P〈0.05).Plasma LPA count increased with the number of diseased coronary branches(F=9.69,P〈0.05).It was found that high LPA count was associated with high coronary artery stenosis scores(F=53.37,P〈0.05).By spearman correlation analysis,the integrated scores of coronary artery stenosis were positively correlated with LPA(r=0.532,P〈0.01).Multiple stepwise regression analysis indicated LPA count were associated with the severity of coronary artery lesions after adjustment for potential confounding variables. Conclusion Advanced LPA level is related to the severity of coronary artery disease.
