摘 要:目的观察成纤维细胞生长因子-2(FGF2)在糖尿病大鼠深Ⅱ度烫伤创面组织中的表达,探讨外源性FGF2应用对创面愈合的影响。方法18只SD大鼠随机分为单纯烫伤组(对照组)、糖尿病烫伤组(糖尿病组)和糖尿病烫伤FGF2干预组(FGF2组)(n=6)。糖尿病组和FGF2组大鼠先经链脲佐菌素诱导建立糖尿病模型后再制备深Ⅱ度烫伤模型,FGF2组大鼠建模后给予创面局部皮内注射FGF2。烫伤后3、7、14 d记录创面未愈合面积并计算创面愈合率;组织学检查观察创面愈合情况;流式细胞仪检测创面细胞周期分布;免疫荧光双标记和双抗体夹心法检测创面组织FGF2表达。结果烫伤后14 d,FGF2组的创面愈合率显著高于糖尿病组,为(53.14±11.44)%vs(25.40±3.00)%(P<0.01),FGF2组与对照组比较差异无统计学意义;与糖尿病组比较,FGF2组创面坏死胶原和G0/G1期细胞比例明显减少。与对照组比较,糖尿病组创面组织释放FGF2减少,为(74.500±13.115)ng/mLvs(100.078±17.863)ng/mL(P<0.05)。糖尿病组创面组织中FGF2和晚期糖基化终末产物(AGEs)共表达。结论糖尿病创面难愈与局部FGF2的数量和功能缺陷有关。创面局部应用外源性生长因子FGF2,可满足糖尿病创面修复细胞对功能性生长因子的需求,促进创面愈合。Objective To observe the expression of fibroblast growth factor-2 ( FGF2 ) in the tissues of second-degree scald in diabetic rats, and investigate the effects of FGF2 on wound healing process. Methods Eighteen SD rats were randomized into 3 groups: scald group (control group), diabetic scald group ( diabetic group) and diabetic scald with FGF2 treatment group (FGF2 group) (n =6). Rats in diabetic group and FGF2 group were treated with streptozotocin to induce diabetes, and second-degree scald models were established afterwards. After model establishment, local intradermal injection of FGF2 in the wound were performed in FGFz group. The unhealed areas of wound were recorded and would healing rates were calculated 3, 7 and 14 days after scald. Healing process was evaluated by histological examination, cell cycle distribution was determined by flow cytometer, and expression of FGF2 was detected by immunofluorescence double labelling and ABC-ELISA. Results Fourteen days after scald, the healing rate in FGF2 group was significantly higher than that in diabetic group, (53.14 ± 11.44) % vs (25.40 ± 3.00) % ( P 〈 0.01 ) , while there was no significant difference between FGF2 group and control group. Compared with diabetic group, the proportions of necrotic collagen and cells in G0/G1 stages were decreased in FGF2 group. Compared with control group, the expression of FGF2 was lower in the tissues of scald in diabetic group, ( 74. 500 ± 13.115 ) ng/mL vs ( 100. 078 ± 17. 863 ) ng/mL ( P 〈 0.05). There was co-expression of FGF2 and advanced glycosylation end prodrcts (AGEs) in the tissues of scald in diabetic group. Conclusion The unfavourable healing process is related to decreased number and functional defect of FGF2 in diabetic wound. The topical use of FGF2 may satisfy the needs of functional growth factors to restore effective wound healing in diabetic rats.
