甄氏外瓶霉致暗色丝孢霉病

摘　　要：报告1例甄氏外瓶霉感染所致的暗色丝孢霉病。患者男,59岁。右小腿外踝上方肿块伴瘙痒5个月,皮损为一直径约3 cm的暗红色斑块。皮损组织病理PAS染色见分隔菌丝,组织块马铃薯葡萄糖琼脂(PDA)培养基上25℃培养长出黑色酵母样菌落,分离菌株最高生长温度37℃,小培养下见分枝分隔菌丝,环痕产孢,分生孢子聚集在环痕梗顶端,分离菌株ITS1区测序结果与甄氏外瓶霉98.54%同源。根据分离菌株生物学特点,菌株被确定为甄氏外瓶霉。A case of phaeohyphomycosis caused by Exophiala jeanselmei is reported. The patient was a 59-year-old male, presented with an itching mass on the upper side of ankle of his right leg for about five months. Physical examination revealed a dark red, ill-defined plaque of 3 em in diameter. Histopathology of the skin specimen showed PAS stained septate hyphae. The tissue euhure revealed black yeast-like colonies with wet surface outgrowth on potato-dextrose-agar medium at 25℃. The highest temperature allowing the isolated strain to propagate was 37℃. Microscopic examination showed annellides with eonidia formed at the apices of the annellophore as well as intercalary loci of the hyphae. The gene sequencing on the ITS1 region showed a homology of 98.54% between the isolated strain and Exophiala jeanselmei. The isolated strain was identified as Exophiala jeanselmei. So, the diagnosis of this case is phaeohyphomyeosis caused by Exophiala jeanselmei.
