摘 要:目的探讨条件免疫反应(CIR)对小鼠支气管哮喘的疗效。方法选取4~6周龄BALB/C雌性小鼠,建立支气管哮喘模型。将模型小鼠分为7组,A组:CIR1组,以声音为条件刺激(CS),布地奈德加沙丁胺醇雾化吸入为非条件刺激(UCS),二者结合7次(7 d)建立CIR,此后每日再予CS,每周CS再与UCS结合1次,共20周;B组:CIR2组,以糖精水为CS,其余同CIR1组;C组:常规治疗组,布地奈德和沙丁胺醇雾化吸入治疗,1次/d,共20周;D组:常规治疗减量组,布地奈德和沙丁胺醇雾化吸入,1次/d,共7 d,之后1次/周,共20周;E组:单纯声音刺激组,声音刺激20周;F组:单纯糖精水刺激组,糖精水刺激20周;G组:空白对照组,PBS雾化吸入20周;H组:健康对照组,用PBS激发20周。治疗后进行肺泡灌洗液(BALF)嗜酸性粒细胞计数;流式细胞术检测其脾脏CD4+T细胞IL-4、IFN-γ及IL-17的表达;HE及MASSON染色观察各组小鼠肺组织病理改变。结果A、B、C组与G组比较:BALF中嗜酸性粒细胞明显减少(Pa<0.01);CD4+T淋巴细胞表达IL-4及IL-17均明显降低(Pa<0.01),IFN-γ增高(Pa<0.01);D、E、F、G组与H组比较,呼吸道上皮增厚,管腔狭窄,管壁周围有大量炎性反应细胞浸润,胶原纤维显著增多;A、B、C组经治疗后呼吸道上皮及炎性浸润情况明显改善,胶原纤维减少。结论CIR可通过调节CD4+T淋巴细胞功能,减少呼吸道上皮增厚、炎性细胞浸润及胶原纤维增生,达到治疗小鼠哮喘的目的。以声音或糖精水为CS,以布地奈德加沙丁胺醇为UCS建立起的CIR,可对哮喘小鼠达到相同的治疗效果。Objective To explore the therapeutic effect of conditioned immune response(CIR) on bronchial asthma in mice. Methods An experimental asthma modal was reproduced on healthy BALB/C mice( female,4 -6 weeks old). After that, all mice were divided into 7 groups randomly and treated as follows:group A(CIR1 group) and group B( CIR2 group) established CIR by receiving the combination of conditioned stimulus(CS) and unconditioned stimulus(UCS) 7 times (7 days). The former used noise as CS and the latter used saccharin (SAC). Both groups nebulised budesonide plus salbutamol as UCS. Then, they were received CS once a day and the combination of CS and UCS once a week for 20 weeks. Group C nebulized budesonide and salbutamol once a day for 20 weeks. Group D nebulized budesonide and salbutamol once a day for the first 7 days, and then once a week for 20 weeks. Group E suffered noise once a day for 20 weeks. Group F were received SAC once a day for 20 weeks. Group G nebulized phosphate buffered solution(PBS) once a day for 20 weeks. Group H nebulized PBS once a day for 20 weeks. After therapy, the bronchoalveolar lavage fluid(BALF) were collected to count eosinophils(EOS) ,the spleens were collected to pick up CD4^ + T lymphocytes, the expression of IL -4,IFN -γ and IL - 17 were detected, and the lungs were taken to observe the pathological changes by HE and MASSON staining. Results The percentages of EOS of group A, B and C were much lower than that of group G ( Pa 〈 0.01 ). The expression of IL - 4 and IL - 17 of group A, B and C were much lower than that of group G( Pa 〈 0.01 ). The expression of IFN - γ in group A, B and C were much higher than that of group G ( Pa 〈 0.01 ). The epithelium tissue of airway of group D, E, F and G was much thicker, the lumens was much narrower,inflammatory cells and collagen fibers were much more than group H,and after therapy, that of group A, B and C were much better. Conclusions CIR can regulate the immunological functions
