摘 要:目的:探讨2型糖尿病(type 2 diabetes mellitus,T2DM)合并代谢综合征(metabolic syndrome,MS)组分与慢性肾脏病(chronic kidney disease,CKD)的相关性。方法:对914例T2DM患者资料进行回顾研究,分析MS各组分及其它危险因素与CKD的关系。结果:(1)358例患者诊断为CKD,患病率为39.17%。(2)CKD组的年龄、总胆固醇、三酰甘油(TG)、低密度脂蛋白(LDL)、尿酸、糖尿病病程均显著高于非CKD组(P<0.01)。(3)有高血压、冠心病、吸烟病史的患者CKD的患病率明显增高(P<0.01)。(4)伴有MS组分的T2DM患者CKD的患病率明显高于不伴有MS组分的患者(40.90%比17.65%,P<0.01)。(5)T2DM患者中,合并MS组分越多,发生CKD的危险性越高(P<0.01)。(6)收缩压(SBP)、TG、LDL、年龄和糖尿病病程是T2DM患者中CKD发生的独立危险因素(P<0.05)。结论:MS及其组分与CKD的发生、发展有着密切关系。SBP、TG、LDL、年龄和糖尿病病程是T2DM患者中CKD发生的独立危险因素。Objective:To study the association between the components of metabolic syndrome and chronic kidney disease in type 2 diabetic mellitus patients. Methods: A retrospective study was conducted with 914 type 2 diabetic mellitus patients and the association between the components of metabolic syndrome and chronic kidney disease in type 2 diabetic mellitus patients was then analyzed. Results: (1) 358 patients was diagnosed CKD, and the prevalence was 39.17%. (2) Elevated levels of total cholesterol, serum triglyceride, LDL cholesterol, uric acid and duration diabetes were significantly associated with increased prevalence of chronic kidney disease (P〈0.01). (3) The prevalence of CKD was significantly higher in patients with hypertension, coronary heart disease and smoking (P〈0.01). (4) The patients with components of MS had a significantly higher prevalence of CKD than that without components of MS (40.90% vs. 17.65% ,P〈0. 01). (5) The risk of CKD increased along with the accumulation of components of MS compared with the simple hyperglycemia (P〈0.01). (6) Systolic blood pressure (SBP), serum triglyceride, LDL cholesterol, age and duration diabetes were independent risk factors associated with CKD in type 2 diabetic mellitus patients(P〈0. 05). Conclusion: Metabolic syndrome and its components are significantly associated with CKD in type 2 diabetic mellitus patients. SBP, serum triglyceride, LDL cholesterol, age and duration diabetes were independent risk factors associated with CKD in type 2 diabetic mellitus patients.
