实时三维超声心动图评价轻度高血压病患者左心功能

摘　　要：目的应用实时三维超声心动图技术评价轻度高血压病患者左心功能。 方法分别对50例正常人和81例高血压患者进行二维超声心动图及三维超声心动图检查。测量左室舒张末期容积、收缩末期容积和射血分数，左房收缩末容积。 结果二维超声测量的左室舒张末容积、收缩末容积、射血分数高血压病患者与正常组间无统计学差异。实时三维超声心动图测值与正常组比较，高血压组左室收缩末容积减低，左室射血分数升高。二维超声心动图测量左房前后径高血压组与正常组无统计学差异。实时三维超声心动图测量左房收缩末容积高血压组较正常组增加。 结论实时三维超声心动图技术较二维超声心动图更能够早期发现高血压病患者左心功能及结构的变化。Objective Using real time three dimensional echocardiography （ RT 3DE） to assess left ventricular function in patients with mild hypertension. Methods Left atrial volume, ejection fraction and left ventricular volume were determined by 2DE and RT-3DE in all subjects. The end diastolic volume （EDV）, end systolic volume （ESV） of the left ventricle and end systolic volume （ESV） of the left atrial （LAESV） were measured. Results LVEDV, LVESV, LVEF by 2DE were not different between normal subjects and patients with mild hypertension. LVEF by RT 3DE in patients with mild hypertension were greater than normal subjects, while LVESV was smaller. Diameter of LA was similar between normal subjects and patients with mild hypertension. Compared with control group, LAESV by RT 3DE increased in patients with mild hypertension. Conclusions The application of real time three dimensional echocardiography presents an important and sensitive tool for detecting left ventricular function and structure in the patients with mild hypertension.
