Quantitative Assessment of Left Ventricular Regional Diastolic Function in Patients with Hypertension Using Strain Rate Imaging

摘  要:目的探讨应变率超声成像(SRI)技术定量评价高血压病患者左室局部舒张功能的临床价值,以期发现评价左室局部舒张功能的有效指标。 方法获取原发性高血压病25例及对照组20例应变率曲线,分别测量各节段舒张早期峰值应变率(SRe)、舒张晚期峰值应变率(SRa)及从QRS波群起点至SRe的时间(CVTSRe);彩色M型多普勒测量左室舒张早期血流播散速度(FPV)。 结果高血压组各节段平均峰值应变率(mSRe、mSRa)及mSRe/mSRa均低于正常组(P〈0.01),mSRa、CVTSRe高于正常组(P〈0.01);mSRe、mSRe/mSRa及CVTSRe同FPV均明显相关(r=0.81,P〈0.001;r=0.62,P〈0.001;r=0.70,P〈0.001),而SRe呈高度显著正相关。 结论高血压病患者SRe的减低同左室整体舒张功能异常显著相关,SRe可作为评价左室局部舒张功能的一个有用而敏感的指标。Objective To explore the clinical values of strain rate imaging (SRI) in quantitative assessment of left ventricular regional function in patients with essential hypertension and find a useful index to assess the regional diastolic function. Methods Strain rate curves were obtained in 25 patients with hypertension and 20 control subjects, and peak early diastolic strain rate (SRe), peak late diastolic strain rate (SRa), and the time from QRS to SRe were measured. The flow propagation velocity (FPV) was measured using color M-mode Doppler echocardiography. Results Compared with normal control group, FPV, mSRe, mSRe/mSRa were reduced and the mSRa, CVTSRe were significantly greater in patients with hypertension; mSRe, mSRe/mSRa and CVTSRe were significantly correlation with FPV, but the best correlation was observed in SRe. Conclusions In patients with hypertension, the reduction of SRe was more closely associated with global diastolic dysfunction, and the SRe may be a useful and sensitive index for regional diastolic fuction.
