摘  要:目的采用定量组织速度成像(QTVI)定量评价各期高血压病患者右心收缩功能的变化。 方法按照WHO标准将高血压病组分为Ⅰ、Ⅱ、Ⅲ期3组,每组各15例,共45例。正常对照组20例。用QTVI技术及单平面面积-长度法、肺动脉频谱法分别测定高血压病各组的右心收缩功能,比较各组间的变化。 结果(1)单平面面积-长度法及肺动脉频谱检测发现仅高血压病Ⅲ组右室收缩功能减低,Ⅰ、Ⅱ组右室收缩功能未见改变;(2)QTVI技术发现高血压病Ⅰ组三尖瓣环的运动速度(Vs)较对照组显著增加,高血压病Ⅱ、Ⅲ组三尖瓣环的Vs均较对照组减低(7.35cm/s、7.53cm/s vs 8.44cm/s),且有逐渐减低趋势。 结论QTVI能够定量检测不同分期的高血压病患者间右心收缩功能的损害程度,其敏感性和准确性均高于常规超声。Objective To evaluate right ventricular systolic function in patients with essential hypertension by Quantitative tissue velocity imaging (QTVI) . Methods According to WHO standard, all the hypertensives were classified into three groups: 15 cases in each group, forty-five hypertensives were enrolled. Twenty age- and sexmatched normal subjects served as controls. By QTVI, two-dimensional and Doppler echocardiography, right ventricular systolic measurement were obtained. To evaluate the difference of parameters between cases and control group. Results The Uniplanar area-length method and pulmonary artery blood flow showed RV global myocardial dysfunction only in group Ⅲ; However, QTVI showed increase of Vs in group Ⅰ and reduction of Vs in group Ⅱ ,Ⅲ (7.35cm/s, 7.53cm/s vs 8.44cm/s) . Conclusions Right ventricular systolic dysfunction with essential hypertension altered aecorg to the degree of staging. QTVI has a greater predictive value than conventional imaging, and is useful for evaluating right ventricular function in patients with essential hypertension.
