结节性甲状腺肿与甲状腺腺瘤的超声造影对比分析

摘　　要：目的对结节性甲状腺肿及甲状腺腺瘤的超声造影进行对比分析,初步探讨超声造影在两者鉴别诊断中的应用价值。方法对18个结节性甲状腺肿及甲状腺腺瘤进行超声造影检查,观察其增强廓清特点,通过时间-强度曲线分析,比较不同性质的结节内造影剂始增时间(AT)、达峰时间(TTP)、峰值强度(PI)参数的差别。结果11个结节性甲状腺肿超声造影后7个结节早于周围腺体组织增强,廓清时间与周围腺体相等;4个结节增强廓清时间与周围腺体相等。7个甲状腺腺瘤超声造影后均早于周围腺体增强,晚于周围腺体廓清。两者在达峰时间、峰值强度上比较差异具有显著性意义(P<0.05)。结论超声造影为结节性甲状腺肿及甲状腺腺瘤的诊断及鉴别诊断提供了更多的信息,有一定的诊断价值。Objective To investigate the value of contrast - enhanced ultrasonography in the differentiation of nodular thyrocele and thyrema by comparing （the two of） their characteristics. Methods Eighteen thyroid nodules were examined by contrast - enhanced ultrasonography. The dynamic enhancement and disappearance pattern of each nodules were observed and the time - intensity curves were analyzed. The parameters incluede enhanced time, time to peak and peak intensity. The data of nodular thyrocele and thyroma were compared. ResultsAfter SonoVue administration, in the 11 nodular thyrocele cascs,7 cases showed early wash in and wash out at the same time;4 cases showed wash in and wash out at the same time in comparison with the peripheral thyroid tissues. Thyroma in 7 cases showed early wash in and wash out in comparison with the peripheral thyroid tissues. Nodular thyrocele and thyroma had statistically differences in enhanced time and peak intensity （ P 〈 0. 05 ）. Condusion Contrast-enhanced ultrasonography can provide more information for the differentiaton of nodular thyrecele and thyroma.
