摘  要:目的比较BD公司BACTEC PLUS+血培养瓶(BD-P瓶)与法国生物梅里埃公司BacT/Alert FA血培养瓶(下简称Bio-F瓶)对含抗菌药物血样中细菌的检测性能。方法将含菌液、抗菌药物、无菌血共5 ml分别注入BD公司BACTEC Standard 10标准瓶(下简称BD-S瓶)、BD-P树脂瓶、法国生物梅里埃公司BacT/Alert SA标准瓶(下简称Bio-S瓶)、Bio-F活性炭瓶的4种血培养瓶内,放入自动化仪器内进行培养,然后记录5 d内阳性瓶的检出时间,实验结束后对4种不同血培养瓶内的检出率及检出时间进行比较。结果含抗菌药物的150瓶中普通瓶仅54瓶被检出,而有吸附能力的150瓶仅32瓶未被检出;BD-P瓶和Bio-F瓶对阿米卡星、氟康唑均有很好的吸附能力,检出时间二者差异无统计学意义,均P>0.05;对替考拉宁Bio-F血培养瓶的检出时间短于BD-P血培养瓶,P=0.009,高浓度的阿米卡星仅Bio-F血培养瓶才检出;而BD-P瓶对于环丙沙星、β-内酰胺类抗菌药物、万古霉素的检出时间明显要短于Bio-F,P值分别为<0.001、0.023和0.009。结论具有抗菌药物吸附能力的培养瓶的检出率要明显高于普通血培养瓶,检出速度明显提高;BD的树脂瓶对绝大多数抗菌药物的吸附能力优于生物梅里埃的活性炭瓶。OBJECTIVE To compare the ability of Becton Dickson BACTEC 9240 PLUS (BD-P) bottles and BioMerieux BacT/Alert FA(Bio-F) bottles in detecting bacterial pathogens in samples containing antibiotics. METHODS Five mililiters of bacteria, antibiotics and sterile blood blend were added into four kinds of blood culture bottles:BD BACTEC Standard 10 (BD-S) bottles, BD-P bottles and BioMerieux BacT/Alert SA(Bio-S) bottles and Bio-F bottles. All the bottles were cultured in automations for five days and then compared the difference of detection rate and the time needed to be positive. RESULTS Blood culture bottles with adsorbing ability had a higher detection rate (118/150) than usual bottles (54/150). Both of BD-P bottles and Bio-F bottles had great ability in adsorbing amikacin and fluconazole and there was no difference of detection time between them (P〉0. 05). BD-P bottles need less detection time in bottles containing ciprofloxacin, β-lactam antibiotics and vancomycin (P 〈 0. 05). But Bio-F bottles were superior in detecting bottles containing teicoplanin and high concentration of amikacin bottles could be detected out in Bio-F bottles. CONCLUSIONS Blood culture media that can adsorb antibiotics have a higher positive rate than common blood culture media and need shorter time to detect bacterial pathogens. The Becton Dickson BACTEK PLUS system is superior to the BacT/Alert FA system in adsorbing most of antibiotics.
